血管免疫母细胞性淋巴结病P53蛋白表达及DNA含量分析

摘　　要：目的研究Ｐ５３蛋白在血管免疫母细胞性淋巴结病（ＡＩＬ）中的表达及ＤＮＡ含量，探讨ＡＩＬ的发生发展规律、病变性质。②方法应用免疫组化技术检测２０例ＡＩＬ病人的免疫表型及Ｐ５３蛋白表达，并用图像分析技术测定ＤＮＡ含量，同时选择非何杰金淋巴瘤（ＮＨＬ）２０例及淋巴结反应性增生病人１０例作为对比研究。③结果ＡＩＬ中Ｐ５３蛋白阳性表达率为３０％（６／２０），与预后无关。ＤＮＡ含量改变对于病人预后有显著影响（ｔ＝２．８６４，Ｐ＜０．０５）。Ｐ５３蛋白表达、ＤＮＡ含量在ＡＩＬ与ＮＨＬ间差异均有显著性（χ２＝４．９１２，ｔ＝３．５５２，Ｐ＜０．０５）。ＡＩＬ中Ｐ５３蛋白阳性表达者ＤＮＡ含量显著高于阴性表达者（ｔ＝２．２２６，Ｐ＜０．０５）。④结论Ｐ５３蛋白表达及ＤＮＡ含量变化均与ＡＩＬ发生发展及恶性转化有关，ＡＩＬ可能为不同原因、不同性质的疾病共有的临床症候群。\ Objective \ To investigate the expression of P53 protein and DNA content in angioimmunoblastic lymphadenopathy(AIL) and discuss the nature and pathogenesis of AIL.\ Methods \ Immunogenotype and P53 protein expression were detected by immunohistochemistry technique, and DNA content was determined by image analysis in 20 cases of AIL. 10 cases of reactive lymphoproliferation and 20 cases of nonHodgkin's lymphoma(NHL) were studied as controls.\ Results \ The positive rate of P53 protein in AIL was 30%(6/20), which had no correlation with the prognosis of patients. DNA content was significantly associated with the prognosis of AIL patients(t=2.864, P<0.05). The positive expression of P53 protein and DNA content showed significant difference between AIL and NHL (2=4.912, t=3.552, P<0.05). DNA content in P53 protein positive cases were significantly higher than that of P53 protein negative cases(t=2.226, P<0.05).\ Conclusions \ P53 gene and DNA content were involved in the pathogenesis and malignant transformation of AIL. AIL may be a kind of clinical syndrome caused by different diseases.\;
