经皮小切口截骨小腿延长术

摘　　要：目的研究经皮小切口截骨小腿延长术手术方法，简化手术，减轻手术创伤及术后肢体反应，缩短延长区骨愈合时间。方法将传统的大切口充分显露骨干截骨改为经皮肤小切口截骨而不显露骨干；将骨刀截骨，截骨后穿针固定改为先穿针固定后用线锯截骨；将近端一组针固定改为两组针固定；将术后５～７天延长改为术后２周延迟延长。结果３３个肢体均达到预定延长长度，延长４～１０ｃｍ，平均６．６ｃｍ；延长区骨愈合时间３～８个月，平均５．６个月；每延长１ｃｍ骨愈合时间为２０～３２天，平均２３天。结论手术切口小，创伤小，骨膜剥离少，解剖对位好，对局部血供保护较好，明显缩短了延长区骨愈合的时间。ObjectiveTo describle the technique of percutaneous osteotomy via minimal skin incision for leg lengthening. MethodsPercutaneous osteotomy via minimal skin incision was performed to substitute the conventional open transverse osteotomy in 33 limbs. ResultsIn all of the 33 limbs, the lengthening obtained was 4 to 10 cm (mean 6.6 cm).The time for bone healing was 3 to 8 months(mean 5.6 months). The healing index was 20 to 32 days (mean 23 days). ConclusionThe minimal skin incision and small area of periosteum detachment caused mild trauma and preserved the local blood supply. As a result, the duration of bone healing was shortened.
