摘  要:目的探讨含ω-3鱼油脂肪乳的肠外营养(PN)支持对全胃切除术后患者营养、肝肾功能及感染性并发症发生率等的影响。方法将88例胃癌患者随机分为对照组和治疗组各44例。在术后的肠内营养支持治疗中,两组均给予普通脂肪乳,治疗组加用ω-3鱼油脂肪乳。于手术前1 d和术后第6天分别测定患者的总蛋白(TP)、清蛋白(ALB)、前清蛋白(PA)、视黄醇结合蛋白(RBP)、转铁蛋白(TF)、血清总胆固醇(TC)以评价营养状况;测定胆红素(T-BiL)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)和血肌酐(Cr)等以评价肝肾功能;比较两组患者术后感染性并发症发生率。结果经过术后6 d的肠外营养治疗,两组之间的营养状况、肝肾功能比较差异无显著性(P>0.05)。与对照组比较,治疗组的感染性并发症发生率较低。结论使用普通脂肪乳与使用含ω-3鱼油脂肪乳作为对全胃切除术后患者的术后肠外营养支持,对其营养状况和肝肾功能的影响无明显差别,而使用含ω-3鱼油脂肪乳的患者感染性并发症的发生率有所降低。Objective To study the effect of parenteral nutritional support containingω-3 fish oil fat emulsion on total gastrectomy patients after operation in aspects such as nutrition,hepatic and renal function,incidence rate of complications,etc. Methods 88 cases of gastric cancer patients were randomly divided into normal control group(giving low calorie parenteral nutrition treatment) and treatment group(applying extraω-3 fish oil fat emulsion additionally).The total protein(TP),albumin(ALB),prealbumin(PA),retinol-binding protein(RBP),transferrin(TF),serum total cholesterol(TC) in patients on day 1 before operation and day 6 after operation were analyzed to estimate their nutritional status.The total bilirubin(T-BiL),alanine aminotransferase(ALT),aspartate aminotransferase(AST) and serum creatinine(Cr) were analyzed to estimate the hepatic and renal function.The incidence rates of infectious complications as well as postoperative nutritional status were compared between the two groups. Results After six days' postoperative nutrition treatment,nutrition status,hepatic and renal function between the two groups had no statistical significance(P0.05).In addition,compared with the control group,the incidence rate of infectious complications in treatment group decreased. Conclusion Comparing ordinary fat emulsion andω-3 fish oil fat emulsion as a parenteral nutrition support for total gastrectomy patients,their nutritional status,hepatic and renal function has no significant difference,while ω-3 fish oil fat emulsion reduced the incidence rate of infectious complications.
