高频彩超对小儿肠系膜淋巴结炎的诊断价值

摘　　要：目的:探讨高频彩超对小儿肠系膜淋巴结炎的诊断价值。方法:对107例以腹痛或反复腹痛就诊或临床拟诊为肠系膜淋巴结炎的患儿进行高频彩超检查。检查时患儿取仰卧位,用高频探头扫查腹腔,观察右下腹、脐周、右中上腹及左侧腹腔。扫查时围绕脐周,在全腹部有序地来回滑动式扫查,重点探查腹痛部位,仔细扫查腹主动脉及下腔静脉等大血管旁。对较肥胖患儿必要时探头可适当加压,以增加探查深度,可提高对肿大淋巴结的检出率。对探查到的肿大淋巴结进行重点观察,并做对比分析。结果:其中102例探查到肿大淋巴结,5例未探查到肿大淋巴结,检出率95.3%。结论:高频彩超对患儿肠系膜淋巴结肿大的检出率高,能为临床诊断肠系膜淋巴结炎提供可靠诊断依据,而且此检查方法直观、安全、方便、无痛苦、无创伤、可重复性好。因此,高频彩超对小儿肠系膜淋巴结炎有重要的诊断价值,是临床诊断小儿肠系膜淋巴结炎重要检查之一。Objective ：To investigate the high- frequency color Doppler uhrasonography in children with the diagnosis of mesenteric lymphadenitis. Me（hods ： 107 eases of abdominal pain or reeurrem abdominal pain or clinical treatment to be diagnosed as mesenterie lymphadenitis in children were examined with high-frequency color Doppler ultrasound examina- tion from May 2007 to March 2009 in our hospital. In inspection, children were tested at supine position, using high-fre- quency abdominal scanning probe to observe the right lower abdomen, Cullen, right in the upper abdomen and left abdom- inal cavity. When scanning around the Cullen in the whole abdomen, back and forth sliding orderly scanning, focusing on abdominal exploration site, carefully scanning abdominal aorta and inferior vena eava and other large blood vessel near. The more obese children, when necessary, the probe can be properly pressurized, in order to increase detection depth, can improve the detection rate of lymph nodes. Swollen lymph nodes on the probe to the carry out key observations and do comparative analysis. Results： 102 eases were detected with swollen lymph nodes, 5 patients without detectable swollen lymph nodes, the detection rate of 95.3%. Conclusion：The high-frequency color Doppler ultrasonography of the mesen- terie lymph nodes detection in children with high rates for the clinical diagnosis of mesenterie lymphadenitis will provide a reliable diagnostic evidence, and this visual inspection method was safe, convenient, no pain, no trauma, with good re- peatability . Therefore, the high-frequency color Doppler of mesenteric lymphadenitis in children has important diagnostic value in the clinical diagnosis of mesenteric lymphadenitis in children. It is one of the most important approaches.
