摘 要:目的探讨血清(1-3)-β-D-葡聚糖在深部真菌感染诊断中的临床价值。方法采用显色鲎试验检测22例患者及40例健康人血清(1-3)-β-D-葡聚糖含量,调查其与深部真菌感染诊断的关系。结果深部真菌感染组的血清(1-3)-β-D-葡聚糖水平(133.60±312.85)pg/ml明显高于健康对照组(41.58±18.91)pg/ml,P<0.005;使用80pg/ml为临界值诊断深部真菌感染的灵敏度、特异度、阳性预测值、阴性预测值分别为81.82%、92.50%、85.71%、90.24%;血清(1-3)-β-D-葡聚糖检测诊断深部真菌感染的ROC曲线下面积为0.945,可信区间为0.890~1.001。结论血清(1-3)-β-D-葡聚糖可用于深部真菌感染的辅助诊断。OBJECTIVE To study the clinical value of serum(1-3)-β-D-glucan detection on the diagnosis of deep fungal infection.METHODS The relationship between the concentration of serum(1-3)-β-D-glucan and deep fungal infection were investigated by using chromogenic LAL to detect the content of serum(1-3)-β-D-glucan in 22 patients and 40 normal persons.RESULTS Patient diagnosed as deep fungal infection(133.60±312.85l)pg/m showed a higher serum level of BG than that of control group(41.58±18.91)pg/ml(P0.005).The sensitivity degree,specificity degree,positive predictive value and negative predictive value for BG were 81.82%,92.50%,85.71% and 90.24%,respectively,with 80 pg/ml as critical value.Area under curve(AUC)was 0.945,with confidence interval between 0.890 and 1.001.CONCLUSION Serum(1-3)-β-D-glucan detection can be used as the assistant diagnosis of deep fungal infection.
