摘 要:目的调查急诊重症监护病房短期出现的多药耐药鲍氏不动杆菌基因同源性,为医院感染的控制提供科学依据。方法 4株多药耐药鲍氏不动杆菌痰标本系急诊重症监护病房肺部感染患者,分离自2009年7月25日-8月7日;按《全国检验技术操作规程》要求操作,用全自动微生物分析仪进行细菌鉴定及药敏试验;用脉冲场凝胶电泳对4株鲍氏不动杆菌进行基因分型,明确其是否为同一菌株的克隆。结果 4株痰标本的鲍氏不动杆菌对包括碳青酶烯类在内的多种抗菌药物耐药,经DNA分型分别为A型和B型,非同一克隆株。结论调查结果显示,鲍氏不动杆菌为多药耐药菌株,不具同源性,排除相互传播的可能性;提醒要合理用药,加强标准预防,提高无菌操作的依从性。OBJECTIVE To investigate the homologous characteristics of multidrug-resistant Acinetobacterbauman- nil (MDR-AB) and provide scientific basis for the control of nosocomial infection. METHODS Four strains of MDR-AB isolated from sputum of the patients with lung infection in emergency intensive care unit (EICU) during Jul 25th 2009 to Aug 7th 2009. The bacterial identification and drug susceptibility test were performed. The genotypes of A. baumannii isolates were classified with pulsed-field gel electrophoresis (PFGE) to determine whether they derived from the same clone strain. RESULTS Four isolates from sputum were resistant to multiple antibiotics including carbapenem. The PFGE types identified from four isolates were type A and type B, which did not derive from the same clone strain. CONCLUSION The result shows that A. baumannii is multi-drug resistant, and not close related. Rational application of antibiotics and standard prevention should be strengthened to improve the compliance of aseptic manipulation.
