摘  要:目的了解近4年常见酵母样真菌感染的分布情况及耐药性,为真菌感染的临床用药提供依据。方法常规分离培养真菌,用VITEK-32型全自动微生物分析仪鉴定到种,药敏试验用惠州市阳光公司提供的稀释法药敏检测卡。结果酵母样真菌感染率由2006年的5.6%升至2009年的23.0%,白色假丝酵母菌是临床真菌感染的主要真菌,而光滑假丝酵母菌的构成比由2006年的20.2%增加至2009年的34.0%成为第2位病原菌,白色假丝酵母菌对氟胞嘧啶(5-FC)的耐药率由2006年的36.2%升至2009年的58.0%,且氟康唑和伊曲康唑的耐药率较高。结论临床酵母样真菌感染逐渐增加,对5-FC耐药情况日趋严重,唑类药物耐药率也较高,应加强对临床真菌感染的耐药情况的检测,以指导临床合理用药。OBJECTIVE To study the distribution and drug resistance of common fungal infections in recently 4 years, to provide basis for the clinical application of antibiotics. METHODS Conventional cultivation of fungi was performed. The fungi were identified by using VITEK-32 full-automatic microbial analyzer, the drug susceptibility test was conducted by using dilution method antimicrobial susceptibility test card supplied by Huizhou Sunshine Company. RESULTS The infection rate of Candida was increased from 5.6% in 2006 to 23.0% in 2009, C. albicans was the major fungus for clinical infection, the composition rate of C. glabrata increased from 20.2% in 2006 to 34.0% in 2009, which became the second pathogen. The drug resistant rate of C. albicans to flucytosine (5- FC) was increased form 36. 2% in 2006 to 58. 0% in 2009, and the drug resistant rates to fluconazole and itraconazole were high. CONCLUSION The clinical Candida infections are gradually increasing, the drug resistance to 5-FC is becoming serious and drug resistance to azole antibiotics is also very high. The detection of drug resistance of the clinical fungal infection should be strengthened to guide the rational application of antibiotics in clinics.
