局麻关节镜手术治疗急性痛风性踝关节炎

摘　　要：目的评估局麻关节镜手术治疗急性踝关节痛风性关节炎的疗效。方法 2004年4月-2009年3月共收治15例单纯踝关节痛风性关节炎,全部为男性,年龄35-51岁,平均41岁;病史2周-3年;左踝6例,右踝9例;在局麻下行关节镜下检查清理术。结果采用美国足踝外科协会AOFAS评分系统进行评估,术前为(51.8±6.7)分,术后为(88.6±5.2)分,术前术后评分有统计学意义(P<0.05)。随访6-60月,平均32.6个月,2例复发。结论单纯踝关节发病的痛风性关节炎发病率较低,误诊率较高,局麻关节镜手术治疗可早期明确诊断,而且创伤小,是治疗踝关节痛风性关节炎的有效方法。Objective To evaluate the therapeutic effect of arthroscopic surgery on ankle arthritis due to acute gout under local anesthesia. Methods Arthroscopic surgery of simple ankle arthritis（left ankle arthritis in 6 patients and right ankle arthritis in 9 patients） due to acute gout was performed in 15 male patients（at the age of 35-51 years, mean 41 years） admitted to our department in April 2004-March 2009 for a history of 2 weeks-3 years of ankle arthritis under local anesthesia. Results The outcome of patients was scored with the American Orthopedic Foot and Ankle Society （AOFAS） score system, which was 51.8 ±6.7 and 88.6 ± 5.2, respectively, before and after operation（P〈0.05）. The patients were followed up for 6-60 months（mean 32.6 months） during which recurrence was found in 2 patients. Conclusion The incidence of simple ankle arthritis due to acute gout is low. However, its misdiagnosis rate is rather high. Arthroscopic surgery under local anesthesia is an effective procedure for it with minimal trauma and helps its early diagnosis.
