髋关节镜诊治髋关节结核的价值

摘　　要：目的探讨关节镜技术在髋关节结核诊断和治疗中的作用。方法我科2001年3月-2008年3月通过关节镜治疗髋关节结核16例,根据病变程度分为早期和晚期病变,术前和术后采用Harris评分评价髋关节功能。结果早期患者术前评分50.89±21.12,术后1年评分86.00±10.33,术后症状明显改善,并能正常生活;晚期患者术前评分3.33±2.07,术后1年评分62.15±17.36,虽较术前明显改善,但仍遗留髋关节疼痛和功能部分受限。结论关节镜可有效地对髋关节结核进行诊断和治疗,早期治疗效果较好。Objective To study the arthroscopic diagnosis and treatment of hip-joint tuberculosis. Methods Sixteen patients who underwent arthroscopic diagnosis and treatment of hip-joint tuberculosis from March 2001 to March 2008 in our department were divided into early lesion group and late lesion group according to the severity of their lesion. The function of their hip-joint was evaluated with Harris score system before and after operation. Results The score for the hip-joint function in early lesion group was 86.00 ± 10.33 before operation and 50.89 ±21.12 one year after operation, indicating that the hip-joint function was significantly improved after operation. The patients had a normal life. The score for the hip-joint function in late lesion group was 3.33 ± 2.07 before operation and 62.15 ± 17.36 one year after operation. Although the hip-joint function was significantly improved compared with before operation, the patients had pain at hip-joint and some of the hip-joint functions were limited. Conclusion Arthroscope can be used in diagnosis and treatment of hip-joint tuberculosis, especially for the early lesion.
