摘  要:目的研究新疆地区铜绿假单胞菌临床分离株中产金属β-内酰胺酶流行株基因型的分布和耐药性。方法收集铜绿假单胞菌95株,分别用金属酶IMP、VIM、SIM、GIM、SPM引物进行聚合酶链反应(PCR),对扩增产物阳性进行序列分析。结果共检出8株金属酶阳性菌株,经基因测序确认4株产IMP-6型金属酶,4株产VIM-2型金属酶,其余基因均阴性;产金属酶菌株均耐美罗培南和亚胺培南,且产酶株对头孢吡肟、头孢他啶、哌拉西林等抗菌药物均不敏感,而对氨曲南较敏感。结论新疆地区铜绿假单胞菌的流行株金属酶基因型主要为IMP-6型和VIM-2型;其耐药性强,临床应根据药敏结果选择用药,以达到最佳抗菌效果。OBJECTIVE To study the genotype distribution and drug resistance of metallo-β-lactamases in Pseudomonas aeruginosa in Xinjiang.METHODS The samples of 95 P.aeruginosa isolates were collected from patients.IMP,VIM,SIM,GIM and SPM were determined by PCR.Alignment analysis was used to these positive products.RESULTS IMP-6 was found positive in 4 strains of P.aeruginosa.VIM-2 was in 4 strains.The others were not found in all 95 isolates detected.Imipenem and meropenem resistant strains were identified from all strains producing metallo-β-lactamases.The metallo-β-lactamases producing strains were resistant to commonly uses β-lactam antibiotics of in clinic,such as cefepine,ceftazidime and piperacillin.The strains were sensitive to aztreonam.CONCLUSION IMP-6 and VIM-2 were the major genotypes in P.aeruginosa producing metallo-β-lactamases in Xinjiang.Clinical antibiotics should be according used to the antimicrobial susceptibility tests results.The epidemiology surveillance of the metallo-β-lactamases producing strains must be strengthened.
