美沙酮门诊治疗维持状况影响因素多重对应分析

摘　　要：[目的]寻找影响美沙酮门诊治疗维持状况、导致病人脱失的危险因素。为提高治疗效果提供参考。[方法]通过访谈、收集参加美沙酮维持治疗患者有效病历资料,根据病人维持治疗状况分为完全脱失、部分脱失、未脱失3种类型,采用多重对应分析,寻找影响维持治疗状况的危险因素。[结果]经济来源为社会福利、临时工资,有心理辅导,吸毒方式为注射+口吸,与毒友交往少和无,来门诊交通时间﹤10min,年龄40～50岁,婚姻状态为丧偶与未脱失有关联;经济来源为其他方式(如:自由职业、违法收入等),口吸吸毒,服务行业,来门诊交通时间﹥1h,年龄20～30岁,与部分脱失有联系;经济来源靠借贷,无心理辅导,注射吸毒,与毒友交往多,来门诊交通时间30min～1h,年龄30～40岁间,婚姻状态为离异与完全脱失有联系。[结论]入组年龄20～40岁、无经济来源或经济来源不稳定、吸毒方式为单纯注射、缺乏心理辅导、来门诊交通时间30min以上、与毒友交往多、离异是影响病人坚持美沙酮维持治疗的主要原因。[Objective] To find the risk factors for methadone maintenance treatment（MMT） statues and resulting in patients＇ dropout,so as to provide reference for improving the work efficiency of clinic methadone maintenance treatment.[Methods] Through interviewing and collecting the effective medical information of patients who participating in methadone maintenance treatment,all these cases were divided into three types：completely dropout group,some dropout group and no dropout group according to the situation of adhering to maintenance therapy.The risk factors which impacted on the status of MMT were explored by multiple correspondence analyses.[Results] The income from social welfare,temporary wages,psychological counseling,the way of drug abuse by injection or oral absorption,less or none contact with Friends who still drug abuse,the time to MMT clinic ﹤10 min,40-50 years old,and widowed were associated with no dropout.The income from other ways（such as freedom occupation and asillegal income etc.）,the way of drug abuse by oral absorption,the service industry,the time to MMT clinic 1 h and 20-30 years old were associated with some dropout;The income by lending,nonpsychological counseling,the way of drug abuse by injection,often contact with friends who still drug abuse,the time to MMT clinic in 30 min-1 h,between 30-40 years old and divorced were associated with completely dropout.[Conclusion] The patients between 20-40 years old,no income or the income instability,the way of drug abuse only by injection,non-psychological counseling,the time to MMT clinic ﹥30 min,often contact with Friends who still drug abuse,divorced are the main reason for patients adhere to the MMT.
