双醋瑞因治疗膝关节骨性关节的临床应用及疗效分析

摘　　要：[目的]研究双醋瑞因治疗膝关节骨性关节炎的可行性、安全性及临床疗效。[方法]108例符合美国风湿病学院(ACR)膝OA标准的患者纳入,随机分入双醋瑞因组(治疗组)、双氯芬酸钠组(对照组),用药12周后观察治疗前后临床疗效和综合疗效评分,并且治疗完毕随访4周。[结果]经12周治疗,试验组和对照组主要疗效指标(20m步行)和次要疗效指标(关节触痛、WOMAC骨关节炎指数等)均较治疗前明显改善,但两组之间比较差异无统计学意义;两组不良反应,试验组较对照组低(P﹤0.05);在随访期,双醋瑞因能持续发挥药效。[结论]应用双醋瑞因治疗膝关节OA与双氯芬酸钠疗效相当,但双醋瑞因作用更持久,具有可靠、安全、优效。[Objective] To evaluate the efficacy and safety of osteoarthritis in knees with diacerein and diclofenac.[Methods] A total of 108 patients fulfilling the American College of Rheummatolgy criteria for knee OA were enrolled in 17 weeks.The two groups patients were given diacerein and diclofenac for 12 weeks,and followed up for 4 weeks.[Results] After 12 weeks of treatment,the primary criterion [visual ananlog scale（VAS） assessment of pain on walking for 20 metres] was significantly superior（P﹤0.05） to baseline in both groups.Significant improvement（P﹤0.05） was observed for the secondary criterion,which included tenderness on palpation,Western Ontario and McMaster Universities OA index（WOMAC）.In the follow-up period,there were no obvious changes in the above parameters of the diacerein group,but in the diclofenac group,pain on walking for 20 metres,tenderness on palpation and WOMAC became worsen after withdrawing the drugs for 4 weeks.[Conclusion] Diacerein showed to be as effective as diclofenac in treating patients with knee OA,and coupled with a carry-over effect.But the diacerein is more effective and is reliable,safety and effectiveness.
