摘　　要：目的观察原发性高血压（EH）及左心室肥厚（LVH）患者血浆脑钠肽（BNP）水平以及贝那普利治疗前后LVH的逆转情况,探讨血浆BNP水平可否作为判断LVH与逆转的一项临床生化指标。方法筛选EH伴LVH组患者56例,EH无LVH组患者66例及正常血压对照组36例,予以EH患者贝那普利5~20 mg治疗24周,分别测定左心室质量指数（LVMI）、E/A比值和BNP,并对BNP和LV-MI、E/A比值进行相关性分析。结果 EH患者BNP浓度显著高于对照组（P〈0.01）,且EH伴LVH者BNP浓度明显高于EH无LVH者,差异有统计学意义（P〈0.01）;经贝那普利治疗后EH伴LVH患者组血浆BNP水平明显下降（P〈0.01）,且与LVMI呈显著正相关,与E/A比值呈显著负相关。结论 EH患者血浆BNP水平明显升高,且与LVH密切相关;BNP浓度的测定可望作为判断EH患者LVH与逆转的一个敏感生化指标。Objective To investigate the levels of plasma brain natriuretic peptide（BNP）in essential hypertention（EH）patients with left ventricular hypertrophy（LVH）before and after treatment with benazepril and identify the role of the plasma BNP in the reversal of left ventricular hypertrophy.Methods 122 patients were divided into three group,including 56 EH patients with LVH,66 EH patients without LVH and 36 cases with normal blood pressure.With the treament of 5~20 mg benazepril for 24 weeks,the serum BNP level and the left ventricular mass index（LVMI）,ratio of E/A before and after treament was determined by two dimensional echocardiography.The correlation between the plasma BNP and LVMI,ratio of E/A were analyzed.Results The plasma BNP level in patients with EH were markedly higher than those in control subjects（P0.01）,and it was significantly increased（P0.01）in hypertensive patients with LVH as composed with those without LVH.Benazepril treatment significantly decreased plasma BNP in patients with and without LVH.The plasma BNP positively correlated with LVMI in patients with EH and negatively correlated with ratio of E/A in patients with EH.Conclusion The plasma BNP levels could increase obviously in hypertensive patients,the increase of the BNP level is closely correlate with the LVH,furthermore,and the plasma BNP may be a marker for existence and reversal of LVH.
