摘 要:目的观察针刀松解法对第3腰椎横突综合征大鼠中枢环磷酸腺苷(cyclic adenosine monophosphate,cAMP)、环磷酸鸟苷(cyclic guanosine monophosphate,cGMP)含量的远期影响,探讨针刀松解法的长效镇痛机制。方法将28只SD大鼠随机分为正常组、模型组、电针组和针刀组,后3组复制模型,后两组再于模型复制后14 d分别给予电针和针刀松解治疗,连续治疗2周。模型复制后56 d采用断头法处死动物,快速取出腰段脊髓和下丘脑,采用放射免疫法检测腰段脊髓和下丘脑cAMP、cGMP含量。结果与正常组比较,模型组腰段脊髓、下丘脑中cAMP水平及下丘脑中cGMP水平显著升高(P<0.01);与模型组比较,电针组、针刀组腰段脊髓和下丘脑中cAMP水平显著下降(P<0.05,或P<0.01),而cGMP水平无显著性变化;与电针组比较,针刀组上述指标均无显著性变化。结论针刀松解法长效镇痛作用的发挥,可能与中枢cAMP含量的变化有关。Objective To observe the long-term effect of acupotomy lysis on levels of cyclic adenosine monophosphate (cAMP), and cyclic guanosine monophosphate (cGMP) in lumbar spinal cord and hypo- thalamus in rats with the third lumbar vertebra transverse process syndrome, and to study the long-term analgesic effects of acupotomy lysis. Methods Twenty-eight SD rats were randomly divided into normal group, model group, electroacupunture (EA) group, and acupotomy lysis (AL) group. Rats in the last 3 groups were induced to the model by an operation. Forty days after the operation, rats in EA group and AL group were treated with EA and AL respectively for 2 weeks . Fifty six days after the operation, all rats were decapitated for taking out the lumbar spinal cords and bypothalamus. The contents of cAMP and cGMP in lumbar spinal cords and bypothalamus were assayed by radioimmunoassay. Results Compared with those of normal group, the contents of cAMP in lumbar spinal cords and hypothalamus, and the con- tent of cGMP in the bypothalamus were increased significantly in model group (P〈0.01). Compared with those of model group, the contents of cAMP in the spinal cords and hypothamalus were significantly decreased in EA group and AL group (P〈0.05, or P〈0.01); but there was no significant difference in cGMP contents among normal group, EA group and AL group. Compared with those of EA group, the levels of above-mentioned indicators had no significant difference. Conclusion The long-term analgesic effects of acupotomy lysis may be related to content changes of cAMP in lumbar spinal cords and hypothalamus.
