摘 要:目的:观察阿奇霉素治疗儿童哮喘患儿嗜酸粒细胞趋化因子(Eotaxin)在治疗前后血清和痰液中水平的变化并探讨可能机制。方法:选取哮喘儿童60例,随机分成常规治疗组(A组)和阿奇霉素治疗组(B组),每组各30例,两组患儿均采用GINA方案中布地奈德吸入疗法,B组另加阿奇霉素口服6周,另选择同期健康儿童20名作为正常对照组(C组)。运用ELASA法检测受试者血清和痰液中Eotaxin水平,并比较各组疗效及指标。结果:①治疗前A、B组血清中Eotaxin水平分别明显高于C组(P均<0.01),痰液中Eotaxin水平亦高于C组(P均<0.01);②治疗后B组血清中Eotaxin水平明显低于A组(P<0.05),痰液中Eotaxin水平明显低于A组(P<0.01);③A、B组治疗后血清和痰液中Eotaxin水平均明显低于治疗前(P均<0.01);④B组哮喘控制疗效显著。结论:阿奇霉素治疗儿童哮喘效果显著,可能通过减少哮喘患儿Eotaxin水平发挥其作用。Objective:To investigate the levels of the eotaxin in serum and sputum in children with asthma before and after treated by using azithromycin and its mechanisms.Methods:60 children with asthma were randomly divided into two groups:Routine treatment group(n=30) and azithromycin treatment group(n=30), and another 20 healthy children enrolled in the control group.Serum samples and sputum samples were taken from the patients before and after treated and the controls.Eotaxins in serum and sputum were quantitatively estimated by enzyme-linked immunoabsorbent assay (ELISA),and the therapeutic effects and the biomarker among groups were compared.Results:①Significant higher were observed for eotaxin levels in serum (P0.01),in sputum (P0.01) of two treatment groups before therapy compared to that of the healthy controls; ②The levels of eotaxin in sreum and in sputum of azithromycin treat group were significantly lower than that in the routine treatment group (respectly,P0.05 and P0.01); ③After treated, the levels of eotaxin in sreum and in sputum of two treatment groups were significantly lower than that before treated (Pall0.01); ④Significant asthma control by using azithromycin.Conclusion:Effects of azithromycin treatment of children with asthma are significantly, and the mechanism maybe by blocking the expression of the eotaxin.
