静脉吸毒艾滋病患者102例抗病毒治疗的疗效分析

摘　　要：目的评价静脉吸毒艾滋病患者的高效抗逆转录病毒疗法（HAART）的疗效,探讨贫困的少数民族地区抗病毒治疗经验。方法对2007年7月—2009年6月接受国家免费抗病毒治疗的102例静脉吸毒感染艾滋病毒（IDU/HIV）的患者进行回顾性分析,总结其疗效和存在的问题。结果 102例静脉吸毒感染HIV的患者,在抗病毒治疗12个月的过程中,服药依从性好的患者47例（46.1%）,因各种原因退出治疗队列的患者36例（35.3%）。在坚持治疗的66例（64.7%）患者中,免疫学成功者48例（72.7%）在治疗6个月、12个月时CD4＋T淋巴细胞检测结果与基线资料比较差异均有统计学意义（P〈0.05）,治疗失败者18例（27.3%）在治疗6个月时CD4＋T淋巴细胞无明显变化,在治疗12个月出现下降,与基线时比较差异有统计学意义（P〈0.05）。治疗1~2年的66例患者中有23例（34.8%）血浆病毒载量在检测水平以下。结论吸毒人群抗病毒治疗队列的终止治疗率较高,服药依从性较差,免疫学疗效和病毒学疗效较差。Objective To evaluate the effectiveness of highly active antiretrovial therapy（HAART） in injection drug users（IDUs） with HIV/AIDS and to discuss the experience of HAART in ethnic minority areas.Methods The data of 102 IDUs with HIV/AIDS receiving HAART from July 2007 to June 2009 were retrospectively analyzed to summarize the effectiveness and the existing problems.Results In the course of 12 months HAART,the medicine-taking compliance of 47 cases（46.1%） was good,and 36 cases（35.3%） dropped out for various reasons.Among 66 cases（64.7%） persisted in HAART,immunologic success was achieved in 48 cases（72.7%）.In the 6th and 12th month of treatment,the number of CD4＋ T lymphocyte was significantly increased as compared with that of the basic data（P0.05）.The number of CD4＋T lymphocyte in 18 case（27.3%） with immunologic failure showed no obvious change in the 6th month,and showed decease in 12th month of the treatment（P0.05）.Among the 66 patients receiving the treatment for 1 to 2 years,there 23 cases（34.8%） whose HIV viral load reduced to the undetectable level.Conclusion In the IDUs group,the rate of HAART termination is higher,The medicine-taking compliance is not good.So the immunological and virological effectiveness is relatively poor.
