髂前上棘撕脱骨折的临床治疗分析比较

摘　　要：目的：探讨髂前上棘撕脱骨折的临床特征和治疗措施.方法：将 2006年7月-2009年6月间本院骨科收治并诊断为髂前上棘撕脱骨折的患者34例在患者知情允许的情况下随机分为保守治疗组（简称A组）和手术治疗组（简称B组）各17例,A组采取保守治疗;B组采取硬膜外麻醉,手术切开,柯氏钉张力带内固定或松质骨螺钉内固定,两组均进行康复治疗并随访1年,观察两种方法治疗髂前上棘撕脱骨折的临床效果.结果：A、B两组病例患侧髋功能及缝匠肌功能恢复正常,均无疼痛等不适;A组7例出现患侧髋前上棘较健侧略隆起,B组髋前上棘外形恢复较好;恢复时间上看,B组患者弃拐行走时间平均为5周,A组患者弃拐行走时间平均为8周,两组数据间差异具有统计学意义（P〈0.05）.结论：髂前上棘撕脱骨折采用保守治疗和手术治疗均可达到理想的治疗效果,但手术治疗能达到解剖复位,不影响外观,且患者恢复时间短于保守治疗.qia on purpose to forward to tear off yielded a clinical characteristic and cure measure ： 2006. method will be next june july and 2009 assigned to the fire and diagnosis of orthopedics qia yielded before tearing off on a 34 cases of the privy to the permission of the conservative treatment （ randomly divided into group a group of instruments ） and the group b ） （ short of the 17 example, a group to group b to the conservative treatment ; hard, cut out the operation, tension with the regular or loose screws in the quality and fixed,Two groups to carry out rehabilitation treatment and visitors from one year, two approaches before qia yielded to treatment on a tear off the clinical effect, the result of a, b - two groups of cases is hip to the side seam of the function and the normal function, without pain and discomfort ; a group of seven cases appear before the hips from side to side a healthy yielded a slightly raised before, and b of form the hip yielded to better recover in time, ; b and group of patients take time for five weeks on average, a group of patients and turned to walk on the average time for eight weeks,Data difference between the two sets of statistics point （ p〈 0.05 ） ： qia conclusion before the tearing off with a fracture yielded to treatment and treatment are up to the ideal of a therapeutic effect, but they could reach the autopsy recovery and not affect the appearance and patients back in time the conservative treatment.
