摘  要:目的 系统评价不同置换液流速的血液滤过对多器官功能障碍综合征(MODS)患者预后的影响.方法 通过检索Medline、Embase、Cochrane临床试验数据库、CBM和CNKI等文献数据库,全面收集国内外不同置换液流速的血液滤过治疗MODS患者的随机对照试验(RCT),提取资料和评估方法学质量,而后采用Cochrane协作网RevMan5.0软件进行统计分析.结果 最终纳入9个RCT共276例MODS患者.Meta分析结果显示,HVHF组病死率42.64%,CVVH组病死率54.70%,两组比较差异有统计学意义[OR=0.45,95%CI(0.25,0.79),P=0.006].结论 HVHF与CVVH治疗比较,可以降低MODS患者的病死率,改善MODS患者的预后.Objective To systematically review the effects of high volume hemofihration (HVHF) and continuous venovenous hemofiltration (CVVH) on the patients with multiple organ dysfunction syndrome (MODS). Methods We searched the Cochrane Library, PubMed, Embase, CBM and CNKI. All randomized clinical trials of MODS requiring hemofiltration were included. Data collection and analysis were taken . Meta - analysis were done by using the Cochrane Collaboration's RevMan 5.0 software. Results Nine RCTs, involving a total of 276 MODS patients were included in the review. The mortality rate of HVHF group was 42. 64%, and that of CVVH group was 54.70%. There was significant difference between them [ OR = 0.45,95% CI ( 0. 25,0. 79 ), P = O. 006 ]. Conclusion Compared with CVVH, HVHF can reduce the mortality of MODS and improve the prognosis.
