高血压病不同中医证型左室舒张功能评价指标E/e比值的变化

摘　　要：目的探讨高血压病不同中医证型之间二尖瓣口舒张早期血流峰速(E)与二尖瓣环舒张早期运动峰速(e)的比值变化规律。方法将120例高血压病患者按照中医辨证分为肝火亢盛证组、阴虚阳亢证组、痰湿壅盛证组、阴阳两虚证组,每组各30例,并设30例健康人做为对照组。应用彩色多普勒超声诊断仪,于心尖标准四腔心切面,使用脉冲频谱多普勒(PWD)与组织多普勒显像(TDI)同步分别检测E值、e值,计算E/e比值,并对各组E/e比值进行比较。结果高血压病各中医证型组与对照组比较E/e比值均升高,差异有统计学意义(P<0.01);阴虚阳亢证组、痰湿壅盛证组、阴阳两虚证组与肝火亢盛证组比较E/e比值均升高,差异有统计学意义(P<0.01);阴阳两虚证组与痰湿壅盛证组比较E/e比值升高,差异有统计学意义(P<0.05)。结论 E/e比值可做为高血压病中医证型客观量化评价指标的参考依据。Objective To investigate the change laws of ratio of early diastolic transmitral flow velocity（E） to mitral annular motion velocity（e,E/e ratio） among different TCM pattern types of hypertension.Methods The hypertension patients（n=120） were divided,according to TCM syndrome differentiation,into the group of overabundant liver-fire pattern（group 1）,group of yin deficiency with yang hyperactivity pattern（group 2）,group of phlegm-dampness accumulation pattern（group 3） and group of dual deficiency of yin and yang pattern（group 4,each n=30）.Other 30 normal persons were chosen as the control group.The values of E and e were detected respectively by using color Doppler ultrasound equipment,PWD and TDI.The E/e ratio was calculated and compared among all groups.Results The E/e ratio increased in all TCM pattern groups compared with that in the control group and the difference was significant statistically（P0.01）.The E/e ratio increased in group 2,group 3 or group 4 compared with that in group 1 and the difference was significant statistically（P0.01）.The E/e ratio increased in group 4 compared with that in group 3 and the difference was significant statistically（P0.05）.Conclusion The E/e ratio can be taken as the reference in objective and quantitative assessment indexes of different TCM pattern types of hypertension.
