摘  要:目的:多色荧光PCR技术分析拉米夫定,阿德夫韦和恩替卡韦治疗不同时期HBV DNA耐药变异情况。方法:将所有病例按照服用拉米夫定,阿德夫韦和恩替卡韦进行分组,定期抽取血清,采用多色荧光PCR技术对其HBV进行检测。结果:拉米夫定治疗6个月后,1例发生耐药变异;12个月后耐药率达到18.8%,24个月后高达40.8%;阿德夫韦服用两年后出现少量耐药,三年后耐药率达到9.7%;恩替卡韦服用期间耐药突变很少。结论:拉米夫定,阿德夫韦和恩替卡韦服用一定的时间均会不同程度的出现耐药变异,定期对服药病人进行耐药检测,有利于及时调整治疗方案,增加疗效,减少不必要的浪费,减轻病人负担。Objective:Detect mutations associated with lamivudine,adefovir and entecavir by Multicolor fluorescence PCR.Methods: All cases were divided into three groups according to different drug exposure,HBV mutations were detected by Multicolor fluorescence PCR termly.Results: One case had mutation after treating by lamivudine for 6 months;resistance rate was 18.8% after 12 months,which reached 40.8% after 24 months.There wre a little mutations after treating by adefovir,the resistance rate was 9.7% after three years.Mutations associated with entecavir were rarely detected.Conclusion: Cases treated with lamivudine,adefovir and entecavir will have mutation in different degree after a period.Detecting drug-resistant mutations of patients regularly were important.It was helpful to change the therapeutic scheme in time,increasing curative effect,reducing unnecessary waste.
