摘 要:目的:观察系统性红斑狼疮患者血清补体水平的变化,并探讨其与其他观察指标以及疾病活动的相关性。方法:回顾分析45例系统性红斑狼疮患者的年龄、病程、免疫球蛋白(IgG、IgA和IgM)、超敏C反应蛋白(hs-CRP)、抗ds-DNA抗体、血清补体C3和C4水平等以及抗链球菌溶血素O(ASO)、血沉(ESR)、全血细胞计数(RBC、WBC、PLT和HGB)和疾病活动指数(DAI)等指标,并以25名健康志愿者为正常对照。结果:SLE患者补体C3和C4水平显著低于正常对照者(P<0.001)。抗dsDNA抗体阳性者C3和C4水平均低于而hs-CRP水平高于抗dsDNA抗体阴性者(P<0.001);DAI≥9分者C3和C4水平均低于DAI<9分者(P<0.001),而二者间hs-CRP水平无显著差异(P>0.05)。不同年龄段和不同病程患者hs-CRP、C3和C4水平均无显著差异(P>0.05)。C3水平与C4、RBC和HGB均呈显著正相关,与IgG、WBC、ESR、ASO、hs-CRP和DAI均呈显著负相关(P<0.05),C4水平与DAI呈显著负相关(P<0.05)。结论:补体C3、C4可在一定程度上反映SLE患者病情的变化,C3和C4水平及其变化可以作为判断SLE疾病活动性的重要指标。Objective:To observe the changes of sera complement C3 and C4 levels in patients with systemic lupus erythematosus(SLE),and to explore the correlation between the complement levels and other indicators as well as disease activitity.Methods: Data from 45 patients with SLE was gathered.The indicators including age,disease duration,immunoglobulins(IgG,IgA and IgM),high-sensitivity C reaction protein(hs-CRP),anti-dsDNA antibody(dsDNA Ab),complement C3 and C4 levels,tilter of antistreptolysin O(ASO),erythrocyte sedimentation rate(ESR),complete blood count(CBC: RBC,WBC,Platelet and hemoglobin) and disease active index(DAI) were observed and evaluated,respectively.The data of 25 healthy volunteers was treated as the compared controls.Results: The sera complement C3 and C4 levels in patients with SLE were significantly lower than those in the healthy controls(P0.001).The C3 and C4 levels in the patients with positive dsDNA Ab were remarkably lower,but hs-CRP level was notablly higher,than those in patients with negative dsDNA Ab(P0.001).The C3 and C4 levels in the patients with DAI ≥ 9 scores were markedly lower than those in patients with DAI 9 scores(P0.001),but hs-CRP levels showed less difference between the two groups of patients(P0.05).There was no statistical difference for the hs-CRP,C3 and C4 levels between different age span as well as disease duration,respectively(P0.05).There was positive correlation between C3 level and C4,RBC as well as hemoglobin levels,respectively,but negative correlation between C3 level and IgG、WBC、ESR、ASO、hs-CRP as well as DAI,respectively(P0.05).There was negative correlation between C4 level and DAI(P0.05).Conclusion: The sera complement C3 and C4 levels are able to reflect the disease status in patients with SLE.It is useful for C3 and C4 to be as the indicators of disease activity of patients with SLE.
