摘 要:目的利用仿组织体模研究热沉没效应对射频消融灶的影响。方法在仿组织体模中建立模拟人体内的自然管道系统,在射频消融过程中从管道内引流10℃和37℃冷却液体,并于射频针的两侧布置测温针。边对体模射频消融边观察测量消融灶的径线,并用测温仪记录消融灶温度场中的温度值。结果测温针温度值自消融开始呈上升趋势,射频电极达到预设消融温度后约2.0min,温度值趋于恒定状态直至消融结束。射频消融结束后,射频针模拟管道引流冷却液体一侧消融灶测值明显小于对侧,10℃组和37℃组测值相差为0.1cm。受引流冷却液体的影响,处于对称位置的2号针和3号针,1号针4号针测值相差约5℃~10℃。两组引流冷却液体的1号针测值均低于45℃,但1号针和2号针10℃组的测值低于37℃组。结论流经消融灶旁模拟引流管道内的冷却液体在射频消融过程中热沉没效应明显,对消融灶的大小及其温度场有着显著影响。Objective To study the impact of the heat sink effect on radiofrequency(RF)ablation lesions using a tissue mimicking phantom. Methods To establish a tubing system that mimics human body duct system in a tissuemimicking phantom. During RF ablation,cooling liquids maintained at either 10 ℃ or 37℃ were drained into the tube, and thermodetector needles were placed on both sides of the RF electrode. The diameter of the ablation lesion was then measured during the performance of the RF ablation on the phantom, and the temperature values in the temperature field of the ablation lesions were recorded using thermodetectors. Results The temperature readings in the thermodetector needles increased after starting the ablation. Two minutes after the RF electrode reached the preset ablation temperature, the temperature values remained fairly constant until the completion of the ablation. Afterwards, the tempera ture of the ablation lesion on the side where the RF needle and the simulated tube drainage of cooling liquid were loca ted was characterized by a significantly lower temperature than that on the other side. Interestingly, there was a difference of 0.1 cm in the measurements between the 10 ℃ group and the 37℃group. The needles arranged insymmetric positions,such as the No. 2 and 3 needles and/or the No. 1 and 4 needles, which were affected by the drainage of cooling liquid,had a 5 ℃ to 10 ℃ difference in temperature. The No. 1 needles,which were used in both drainage cooling liquid groups,were lower than 45 ℃ . However,the No. 1 and No. 2 needles in the 10 ℃ group were lower than those observed in the 37℃ group. Conclusions The cooling liquid flowing through the simulated tube proximal to the ablation lesions showed significant heat sink effects during RF ablation,which had significant impacts on the size of the ab lation lesions as well as its temperature field.
