动脉调转术138例的超声心动图近中期随访研究

摘　　要：目的超声心动图随访动脉调转手术(ASO)近中期患者,观察其心脏及大血管结构、功能及血流状况,探讨其超声检查的方法及特点。方法对138例ASO进行了3～57个月随访,平均(27.7±14.6)个月。其中男性108例,女性30例,平均年龄(11.7±26.3)个月。均行全面超声心动图检查。结果对所有患者均完成满意的全面心脏及大血管探查。新生主动脉瓣轻度以上反流15例;肺动脉分支狭窄6例;仍残留肺动脉高压10例;无心功能不全。结论超声心动图随访结果显示ASO手术具有良好的近中期结果,是治疗完全性大动脉转位的理想术式。部分患者的两大动脉相对位置关系非完全正常位置,超声检查手法应多方位、多切面,尤其是对肺动脉的探查。Objective To assess heart structure and function in patients with transposition of the great arteries （TGA） and Taussig-Bing malformation from early and middle term follow-up by echocardiography after arterial switch operation （ASO） . Methods One hundred and thirty-eight patients were reviewed, among which there were 108 males and 30 females. The mean age was 11.7±26.3 months （3±57months） . The operative time was from January 2004 to December 2007. The follow-up was finished from July to December 2008. The mean follow-up periods were 27.7± 14.6 months （3±57months） . Thorough echocardiography exams were performed in all patients. Results Sa tisfying echo views were achieved in all patients. Ninety percent cases were with more than mild neo-AVI. Ninety-{our percent cases were with severe moderate to-severe pulmonary artery stenosis. Significant pulmonary hypertension still existed in 10 cases. Cardiac function was normal in all cases. Conclusions Echocardiographic results of early and middle term follow-up show that ASO is an optimal choice in the treatment of various forms of TGA. The abnormal positions of the neo pulmonary arteries show some challenge in echo exam on some cases.
