摘 要:目的探讨广东汉族人群2型糖尿病和冠心病患者血管内皮舒张功能障碍与内皮型一氧化氮合成酶(endothelial nitric oxidesynthetases,eNOS)基因G894T多态性的关系。方法对175例2型糖尿病和冠心病患者进行eNOS基因G894T多态性分析和肱动脉血管内皮功能超声检测,根据eNOS基因G894T基因型分为GG组和GT组,比较两组间内皮依赖性舒张功能(flow-mediated dilatation,FMD)和非内皮依赖性舒张功能(glyceryl trinitrate-mediated dilatation,GTNMD)。结果 GG组与GT组患者的FMD分别为:(18.15±7.33)%和(14.94±6.19)%,GTNMD分别为:(22.93±9.23)%和(19.42±7.13)%;GG组与GT组FMD、GTNMD之间的差异具有统计学意义(P<0.05),GT组FMD和GTNMD均较GG组差;单因素相关分析及多重回归分析均显示FMD、GTNMD与eNOS基因G894T多态性显著相关,T等位基因是血管内皮舒张功能损伤的独立危险因素。结论广东籍汉族人群2型糖尿病和冠心病患者,FMD和GTNMD与eNOS基因G894T多态性相关,GT型患者的FMD和GTNMD均较GG型患者受损明显。Objective To investigate the association between vascular endothelial dysfunction and endothelial ni tric oxide synthetases (eNOS) gene G894T polymorphisms in patients with type 2 diabetes mellitus (T2DM)and/or coronary atherosclerotic heart disease(CHD). Methods In 175 patients with T2DM and/or CHD,flow-mediated dila tation(FMD, endothelium dependent) and glyceryl trinitrate mediated dilatation ( GTNMD, endothelium independent) of the right brachial artery were studied by using high-frequency ultrasound. ENOS gene G894T polymorphisms was also detected by PCR RFLP with restriction endonuclease Ban Ⅱ . The patients were grouped into GG and GT by their eNOS genetypes. FMD and GTNMD of the two groups were compared. Results FMD (14.94 ± 6.19% vs 18. 15± 7.33%)and GTNMD(19. 42 ±7.13 % vs 22. 93 ±9. 23 % ) were significantly lower in the patients of GT group than those of GG group(P〈0.05). Linear correlation and multiple regression analysis showed that the genetypes of eNOS gene G894T were closely related to FMD and GTNMD and T allele was the independent determinant in inducing the injury of endothelial function in all patients(P〈0.05). Conclusions In patients with T2DM and/or CHD,FMD and GTNMD are related to eNOS gene G894T polymorphisms. FMD and GTNMD are worse in GT carriers than those in GG carriers.
