摘  要:目的探讨经阴道子宫输卵管三维超声造影评价不孕症患者输卵管通畅性的临床应用价值。方法选择不孕症妇女43例,将双腔球囊导管置入宫腔,经导管推注稀释SonoVue超声造影剂(10ml/侧),经阴道超声造影实时观察增强情况,并采用经阴道三维超声造影重建技术评估输卵管通畅性,其中2例行腹腔镜检查,5例行子宫输卵管碘油造影。结果 43例患者的85条输卵管超声造影三维重建显示81条,显示率95.3%;其中输卵管通畅26条,输卵管通而不畅40条,输卵管阻塞15条。结论经阴道子宫输卵管三维超声造影重建技术能真实展现输卵管的立体空间走行,并能准确、快速判断输卵管的通畅性,有望作为一种新的筛查不孕症患者输卵管通畅性的有效手段。Objective To assess clinical value of the fallopian tubes patency by using three-dimensional hystero salpingo contrast sonography ( 3D HyCoSy ) with SonoVue and transvaginal ultrasound ( TVS ) in infertile women. Methods 10 ml diluted ultrasound contrast agent was injected into the cavity of uterus through Foley's tube in forty-three women with infertility. The TVS probe was positioned in such a way as to visualize the transverse section of the uterus. The data of the contrast agents for 3D HyCoSy were obtained during injection of the agents and special soft- ware automatically provided the volume image produced by the agents. Two patients accepted the examination of laparoscopy. And five cases were detected by X-ray hysterosalpingography(X-ray HSG). Results Eighty-one uterine tubes were clearly demonstrated by TVS 3D HyCoSy among which fifteen tubes were diagnosed by HyCoSy to be blocked, forty incompletehy blocked, and twenty-six unobstructed. Conclusions TVS 3D HyCoSy can show the shape of uterine tube in three dimensions. And it can accurately and quickly assess the tubes patency. This technique is hopeful to be come an effective method to diagnose the tube patency in patients with infertility.
