后穹窿穿刺液与外周血中β-HCG比值在不典型异位妊娠中的临床诊断价值

摘　　要：目的:探讨后穹窿穿刺液中β-HCG定量测定对不典型异位妊娠的诊断价值。方法:检测92例因可疑异位妊娠患者血β-HCG及后穹窿穿刺液中β-HCG的含量,并计算2者比值,分析其在不典型异位妊娠中的诊断价值。结果:92例患者中81例经腹腔镜探查证实为异位妊娠,其后穹窿穿刺液中与外周血β-HCG分别为(21746.58±9862.39)mU/mL及(1854.34±998.72)mU/mL,比值为12.43±5.72,其中有1例(1.23%)外周血β-HCG结果在正常范围内;11例经诊断性刮宫术证实为宫内妊娠,其后穹窿穿刺液中β-HCG与外周血β-HCG分别为(16472.63±7892.57)mU/mL和(14937.57±6364.83)mU/mL,比值为1.13±0.63,前者比值远大于后者,差异有统计学意义(P<0.01)。结论:后穹窿穿刺液与外周血β-HCG比值对不典型异位妊娠的诊断有较大的意义。Objective：To discuss the diagnostic value of the quantitative determination about β-HCG in peripheral blood and in fluid punctured from the posterior fornix for atypical ectopic pregnancy.Method：According to the results confirmed by pathology,92 cases of hospitalized patients were divided into the ectopic pregnancy group（81 cases）and the intrauterine pregnancy group（11 cases）.Both β-HCG in the fluid punctured from posterior fornix and the peripheral blood in two groups were quantitatively analyzed.The ratio of between β-HCG in fluid punctured from posterior fornix and that in peripheral blood were also analyzed statistically.Result：The β-HCG level in fluid punctured from posterior fornix puncture fluid and that in peripheral blood in the ectopic pregnancy group are（21 746.58± 9 862.39）mU/mL and（1 854.34±998.72）mU/mL respectively,which have significant differences（P0.05）.The ratio is 12.43±5.72.One case（1.23%）in these group has a negative β-HCG level in peripheral blood.The β-HCG levels in the two site mentioned above were（16 472.63±7 892.57）mU/mL and（14 937.57±6 364.83）mU/mL respectively,which have no significant differences（P0.05）,and the ratio is 1.13±0.63.The ratio between β-HCG in fluid punctured posterior fornix and that in peripheral blood in the ectopic pregnancy group is much higher than that in intrauterine pregnancy group（P0.05）.Conclusion：The ratio between β-HCG in fluid punctured posterior fornix and that in peripheral blood contribute to the accurate diagnosis of atypical ectopic pregnancy.
