维持性血液透析抑郁症患者实施音乐干预的疗效及影响因素分析

摘　　要：目的探讨维持性血液透析(maintenance hemodialysis,MHD)抑郁症患者实施音乐干预的疗效及其影响因素。方法选择2005年7月-2008年7月在本院行MHD的抑郁患者37例,在血液透析过程中对患者实施音乐干预。采用Zung抑郁自评量表(self-rating depression scale,SDS)评价音乐干预前后患者抑郁情绪的改善情况及其影响音乐干预的因素。结果音乐干预后患者SDS总得分明显下降(P<0.05),其中性格外向、≥60岁、文化程度高、婚姻稳定、≤6月透析龄的MHD抑郁患者经音乐干预后疗效较明显(均P<0.05);外向型性格、音乐知识丰富的护理人员为患者实施音乐疗法后,患者抑郁情绪改善较明显(均P<0.05)。结论音乐干预疗法可减轻MHD抑郁患者的抑郁情绪,其中MHD患者的性格、年龄、婚姻、文化程度、透析龄及护理人员的性格和音乐知识等因素是影响患者实施音乐疗法干预效果的因素。Objective To investigate the relevant factors of music therapy in patients with maintenance hemodialysis.Methods Thirtyseven patients with depression were treated by music during maintenance hemodialysis in our hospital from July 2005 to July 2008.All patients were assessed by the Zung Self-rating Depression Scale（SDS） on their depression improvement after music therapy.Relevant factors of music therapy were analyzed.Results After music therapy,the total SDS score of the MHD patients decreased,and the difference was significant（P〈0.05）.Patients with outgoing personality,over 60 years of age,high education level,stable marriage,and less than 6 months of dialysis had significant improvement after music intervention（P〈0.05）.After the implementation of music therapy by nurses with export-oriented personality and musical knowledge,SDS scores of MHD patients with decreased,and the difference was significant（P〈0.05）.Conclusion Music therapy can reduce depression in patients with MHD,and patients＇ character,age,marital status,education level and dialysis age and nurses＇ character and music knowledge can impact the effect of music therapy.
