影响门脉高压患者心得安应答率的护理研究与干预策略

摘　　要：目的从护理学角度分析影响门脉高压患者服用心得安应答率的相关因素，探讨干预对策及干预治疗对提高应答率的影响。方法66例有心得安服用指征的门脉高压患者常规服用3个月后依应答与否分为应答组与无应答组，比较两组的就医行为，对无应答组实施护理干预，改晨间心率监测为晨间与晚间两次静息心率同时达标为用药调整参数，采用多普勒测定门静脉宽度，分析干预前后心率达标率及门静脉宽度变化。结果与应答组相比，无应答者遵医性差，医护对用药宣教欠缺、亲属支持程度低为主要影响因素。加强宣教、改晨间与晚间两次静息心率为用药调整参数后，应答率提高，门静脉宽度明显缩小。结论门脉高压患者服用心得安应答率低与遵医性差有关，加强宣教、特别是取得家属成员的支持配合甚为重要。结合晨间与晚间两次心率监测指导用药调整可提高应答率。Objective To analyze factors influencing inderal responding rates on patients with portal hypertension and to investigate the strategy of nursing intervention. Methods Sixty - six cases of portal hypertension having indication of taking inderal were analyzed after 3 months routine inderal taking. Medical behaviors were studied and compared between the response group and the non -response group. For non - response group, inderal doses were adjusted according to heart rates checked both in the morning and evening. Heart rate response rates and portal vein diameters were compared before and after nursing intervention. Results Forty - one cases accomplished the study. Inadequate medical behaviors in taking inderal were found. Inadequate health education and supports from the relatives were the causes for the incompatibility. After nursing intervention, medical behaviors were improved and response rate was increased with the enlarged portal vein diameter decreasing. Conclusions Inadequate medical behavior may be the cause of the low response rate. Health education and supports from the relatives could improve the response rates, doses adjusted according to hearts both checked in the morning and evening could help to increase the response rates.
