HBV持续感染患者调节性T细胞检测及其意义

摘　　要：目的研究调节性T细胞(Treg)在慢性乙型肝炎(CHB)发病机制中的作用,为寻找治疗CHB的新方法奠定基础。方法用流式细胞仪检测了75例CHB患者和30例慢性乙型肝炎病毒(HBV)携带者外周血中Treg含量,CHB患者中HBeAg阳性患者39例,HBeAg阴性患者36例,慢性HBV携带者中HBeAg阴性和HBeAg阳性患者各15例。并对其中23例CHB患者服用核苷(酸)类似物前后外周血Treg含量进行检测。结果 CHB患者外周血Treg含量明显高于慢性HBV携带者(P<0.05),而CHB和慢性HBV携带者中HBeAg阴性和HBeAg阳性患者外周血Treg含量均无明显差异(P>0.05)。经核苷(酸)类似物治疗12周后,CHB患者外周血Treg含量较治疗前有所降低(P<0.05)。结论 Treg在HBV致病机制中起一定的作用,体外调控Treg含量和功能可能为治疗HBV感染提供一个新的途径。Objective To investigate the role of T regulatory cell（Treg） in the pathogenesis of chronic hepatitis B（CHB） and lay the basis for new therapy of CHB.Methods In 75 patients with CHB and 30 HBV carriers underwent determination of Treg levels in the peripheral blood by flow cytometry,of whom there were 39 HBeAg-positive patients,36 HBeAg-negative patients,15 HBeAg-positive carriers and 15 HBeAg-negative carriers.The Treg levels in the peripheral blood were determined before and after administration of nucleos（t）ide analogues in 23 CHB patients.Results The Treg levels in the peripheral blood in CHB patients were significantly higher than those in the chronic HBV carriers（P0.05）,while there was no significant difference in Treg levels in the peripheral blood between HBeAg-positive and HBeAg-negative CHB patients and chronic HBeAg carriers（P0.05）.12 weeks after nucleos（t）ide analogue therapy,Treg levels in the peripheral blood in CHB patients had a statistical decrease compared with the baseline（P0.05）.Conclusion Treg plays an important role in the pathogenesis of CHB.External regulation of the level and function of Treg might provide a new means for the therapy of HBV infections.
