糖化血红蛋白达标治疗改善初诊2型糖尿病C肽分泌功能的研究

摘　　要：目的探讨初诊2型糖尿病患者接受糖化血红蛋白（HbA1c）≤6.5%达标治疗前后自身C肽分泌功能的变化及意义。方法初诊2型糖尿病患者53例,依据自身C肽分泌水平选择相应的非胰岛素促泌剂药物治疗3-6个月至HbA1c≤6.5%。对治疗前后OGTT-C肽释放试验结果进行比较。结果治疗后0、30、60、120、180min的OGTT血糖值均显著降低（P〈0.01）;30、60、120min的C肽值均显著升高（P〈0.01）;FCP/FPG、△C30/△G30、△C60/△G60、△C120/△G120、C180/G180以及2hCP/FCP均显著升高（P〈0.01）。结论 HbA1c达标治疗能有效增加初诊2型糖尿病患者葡萄糖刺激的C肽分泌,明显改善C肽分泌模式,有利于控制高血糖病情进展。Objective To study the changes and significance of C-peptide secretion function before and after treatment reaching a goal of HbA1C ≤6.5% for patients with newly diagnosed type 2 diabetes mellitus. Methods 53 patients with newly diagnosed type 2 diabetes mellitus received non-insulin secretagogue treatment corresponding to self C-peptide secretion levels for 3～6 months till HbA1C≤6.5%.Compared the outcomes of OGTT-C peptide secreation before treatment with after treatment.Results Compared with before treatment,all of the OGTT blood glucose decreased significantly at time points of 0,30,60,120 and 180 min（P0.01）;C-peptide increased significantly at 30,60 and 120 min time points（P0.01）;the ratios of FCP/FPG,△C30/△G30,△C60/△G60,△C120/△G120,C180/G180 and 2hCP/FCP increased significantly（P0.01）.Conclusion HbA1C targeted therapy（≤6.5%） can significantly stimulate the secretion of C-peptide for patients with newly diagnosed type 2 diabetes mellitus,improve the C-peptide secretion mode and benefit the control of high blood glucose development.
