摘 要:目的探讨不同浓度胰岛素对右室三层细胞即内膜下、中膜、外膜下细胞动作电位(AP)的影响。方法采用酶解法分离右室三层心肌细胞,膜片钳技术记录右室三层细胞的AP,分别予40,280,600,2 400,4 000 pmol/L胰岛素干预后记录动作电位振幅(APA)、完成动作电位复极20%、50%、90%的时间即APD20、APD50、APD90并进行比较。结果①40 pmol/L胰岛素对右室外膜下、中膜细胞APD无明显影响;280,600,2 400,4 000 pmol/L胰岛素显著缩短右室外膜下、中膜细胞APD。②40,280,600 pmol/L胰岛素对右室内膜下细胞APD无明显影响;2 400,4000 pmol/L胰岛素显著缩短右室内膜下细胞APD。③4 000 pmol/L胰岛素使右室外膜下、中膜细胞APA辐值显著减小。结论胰岛素对右室内外膜心肌细胞的AP呈现不一致的作用,增大了跨膜动作电位梯度,可能与心律失常的发生有关。Objective To investigate the heterogeneity of action potential(AP) and the effects of different concentrations of insulin on APs of the three different membranes of rabbit's right ventricle. Methods The sub-epicardial(Sub-Epi) ,midcardial( M ), and sub-endocardial (Sub-endo) cells were obtained by enzymes digestion from right ventricle of rabbits and were intevened by different concentration of insulin at 40,280,600,2 400,4 000 pmlo/L. APs were recorded via whole-cell patch-clamp technique. Results According to current clamp protocol, APs of the three membranes of the rabbits' right ventricle were significantly different. (1)40 pmol/L insulin did not significantly affect AP duration(APD) of Sub-Epi and M cells ;280,600,2 400 and 4 000 pmol/L insulin significantly shortened APDs of Sub-Epi and M cells. (2)Insulin did not have significantly effect on APDs of Sub-endo ceils at the concentration of 280 and 600pmol/L;At the concentration of 2 400 and 4 000 pmol./L, insulin significantly shortened APDs of Sub-Endo. (3)4 000 pmol/L insulin significantly decreased the APA of Sub-Epi and M cells. Conclusions Insulin has different effects on APDs of the three membranes of rabbit's fight ventricle and may increase the voltage gradient of myocyte ,which could be the mechanisms of arrhythmia.
