摘  要:[目的]观察三维动态灌注法构建的大段组织工程化骨修复羊胫骨干节段性骨缺损的效果。[方法]利用自行设计的灌注型生物反应器对复合骨髓基质干细胞的大段磷酸三钙柱状载体进行动态灌注培养或静态培养2周,通过葡萄糖日耗量、乳酸生成量监测细胞在载体内的扩增。在羊胫骨干节段性骨缺损中分别植入β磷酸三钙、静态培养和动态灌注培养的组织工程化骨,通过X线片、大体观察、硬组织切片和形态计量学检查观察其修复骨缺损的效果。[结果]动态培养法葡萄糖日耗量及乳酸生成量明显高于静态培养法。单纯β磷酸三钙、静态培养和动态灌注培养的组织工程化骨植入骨缺损16周后的临床愈合率分别为0/6、2/6、4/6。动态灌注培养组的材料残余率明显低于单存材料组(P<0.05)。[结论]三维动态灌注培养法构建的大段组织工程化骨修复羊长骨节段性缺损的效果优于传统静态培养法。[Objective]To investigate the long bone defect repair in the goats with large tissue engineered bone created by 3-D perfusion culture. [Methods]The porous β-TCP(tricalcium phosphate) cylindrical scaffolds combined with the goat mesenchymal stem cells were perfused dynamically or cultured statically for 2 weeks by a perfusion bioreactor.The cell proliferation was monitored by the daily glucose consumption and lactic acid production.The β-TCP and the engineered bone by static and dynamic perfusion culture were implanted in the tibial shaft defect of the goat.The radiograph,the gross inspection and the undecalcified histological study were performed to evaluate the bone defect repair.[Results]The daily glucose consumption and lactic acid production were much higher in the dynamic perfusion group than in the static group.The healing rates with the β-TCP,the engineered bone by static and dynamic perfusion culture were 0/6,2/6,4/6 respectively at 16 weeks.The residual material in dynamic perfusion culture group was much lower than in the β-TCP group(P0.05).[Conclusion]The engineered bone created by the 3-D dynamic perfusion culture could reconstruct the segmental long bone defect more efficiently than the bone created by the static culture.
