外科围手术期抗菌药物使用分析

摘　　要：目的:加强抗菌药物的合理应用和减少药物滥用。方法:随机抽取本院2009年1—6月行手术治疗的住院病历300份,调查不同切口类型手术围手术期预防使用抗菌药物的情况。对抗菌药物的选择、剂量、给药方法与疗程等进行合理性评价。结果:300份病历中,抗菌药物使用率92.3%,其中Ⅰ类切口使用率72.0%,Ⅱ、Ⅲ类切口使用率100%;抗菌药物不合理应用主要是选药不合理、用药方法不正确、用药疗程不合适及初次给药时间不合适,分别占10.7%、7.7%、6.0%及6.7%。结论:本院外科围手术期抗菌药物应用趋于规范,但还存在选药档次偏高、术后预防应用时间偏长等问题,需要继续加强抗菌药物使用督导工作。Objective To enhance rational use of antimicrobials and decrease drug abuse.Methods The medical records of 300 patients who received operation from January to June,2009 were collected to evaluate the perioperative use of antimicrobials.The selection and dosage of drugs,medication,and course of treatment were evaluated.Results The usage of antimicrobials was 92.3% in 300 cases,72.0% in type Ⅰ incision,100% in type Ⅱ and Ⅲ incisions.The main unreasonable uses of antimicrobials were the irrational selection of antimicrobials,the incorrect method of using antimicrobials,improper course of treatment and the wrong time of administration for the initial time,counting for 10.7%,7.7%,6.0%,and 6.7% respectively.Conclusion Perioperative use of antimicrobials is reasonable in our hospital.But there are a lots to correct and improve,such as the choosing of high-level antibacterials,and prolonged medication duration after operation for prevention.Administration should be enhanced in using antimicrobials.
