摘  要:目的:探讨多层螺旋CT多平面重建技术在诊断卵巢囊肿蒂扭转中的应用价值。方法:回顾性分析15例经手术病理证实的卵巢囊肿蒂扭转患者的临床资料、多层螺旋CT及多平面重建图像表现,并与手术病理结果对照分析。结果:15例多层螺旋CT均表现为卵巢囊性肿块,最大径为8.73 cm。囊壁增厚11例,无增厚4例,平均1.75 cm。增厚的囊肿壁呈非对称性,连续层面观察可见囊壁最厚处有条索状软组织密度的蒂部相连并延伸至一侧附件或一侧子宫角部,多平面重建能更清楚显示蒂部这一特征性表现。9例合并盆腔积液或腹腔积液。结论:卵巢囊肿蒂扭转具有特征性的多层螺旋CT表现,多层螺旋CT及多平面重建在诊断卵巢囊肿蒂扭转中有较高的临床应用价值,多平面重建可弥补轴位CT图像的不足。Objective To investigate the application of multi-slice CT and multi-planar reconstruction in pedicle torsion of ovarian cysts.Methods The clinical data,multi-slice CT and multi-planar reconstruction films manifestations of 15 cases of pedicle torsions of ovarian cysts confirmed by surgery and pathology were retrospectively analyzed and compared with pathological results.Results The multi-slice CT showed cystic mass of ovary in 15 cases,with the mean maximum diameter of 8.73 cm.Eleven cases had thickened walls,and 4 cases did not.The mean wall thickness was 1.75 cm.The thickened wall showed asymmetric,and the greatest thickness parts of it had a striped soft tissue density pedicle connecting with adnexa or uterine horn.Multi-planar reconstruction films could clearly show this characters of pedicle.Nine cases had pelvic effusion or ascites.Conclusion The pedicle torsion of ovarian cysts has some characteristic multi-slice CT findings.Multi-slice CT and multi-planar reconstruction techniques have a high value to the diagnosis of it.MPR could reconstruct images at any plane,so it could make up for the lack of axial CT scanning.
