新生儿窒息生化指标改变临床意义

摘　　要：目的:探讨新生儿窒息生化指标的动态变化的意义。方法:对60例窒息新生儿(依病情分轻、中、重组)和30例正常新生儿分别在24 h内,第7天采集股静脉血,测定血中肌酸激酶心型同工酶、乳酸脱氢酶同工酶-1、血肌酐、皮质醇水平,血糖测定采用末梢血。观察窒息新生儿血生化指标的动态变化并与正常新生儿进行比较。结果:窒息组24 h内血清肌酸激酶心型同工酶、乳酸脱氢酶同工酶-1、血肌酐、皮质醇水平高于对照组(P<0.05),均随病情加重而升高,在病程第7天基本恢复正常。窒息组患儿24 h内血糖变异性大,轻、中度窒息以低血糖为主,重度窒息以高血糖为主,差异有统计学意义(P<0.05)。第7天各组血糖差异无统计学意义(P>0.05)。结论:窒息新生儿血清中肌酸激酶心型同工酶、乳酸脱氢酶同工酶-1、血肌酐、皮质醇水平及血糖变化与窒息程度有密切相关性。Objective To discuss the significance of the changes of creatine kinase MB isoenyme,lactate dehydrogenase-1,serum creatinine,cortisol and plasma glucose in neonatal asphyxia.Methods A total of 60 cases of asphyxia neonates and 30 normal newborns were collected vein blood in 24 hours and the seventh day after birth respectively to determine the total levels of creatine kinase MB isoenyme,lactate dehydrogenase-1,serum creatinine and cortisol.Results The levels of creatine kinase MB isoenyme,lactate dehydrogenase-1,serum creatinine and cortisol in group of asphyxia were higher than those in control group in 24 hours（P0.05）,increased with the aggravation of patient＇s conditions and turned normal after 7 days.In group of asphyxia,plasma sugar changed obviously in 24 hours.The slight and moderate asphyxia groups mainly showed hypoglycemia and severe group mainly showed hyperglycosemia,which showed a significant difference in 24 hours（P0.05） and no significance in the seventh day（P0.05）.Conclusion The levels of creatine kinase MB isoenyme,lactate dehydrogenase-1,serum creatinine,cortisol and plasma glucose are closely correlated to the degree of neonatal asphyxia.
