摘  要:目的:探讨高血压合并糖尿病的老年患者脉压指数与早期肾损伤的相关性。方法:选择分为高血压合并糖尿病患者52例为观察组,单纯高血压患者50例为对照组,对2组进行24 h动态血压监测和尿微量蛋白测定,比较2组脉压、脉压指数及尿微量蛋白水平,分析2组中脉压指数与尿微量蛋白相关性,观察组中脉压指数与糖化血红蛋白相关性。结果:观察组中糖化血红蛋白、脉压及脉压指数较对照组显著升高(P<0.05)。观察组各种尿微量蛋白均高于对照组(P<0.05)。观察组中脉压指数与尿微量蛋白的相关系数均高于对照组。脉压指数与糖化血红蛋白有正相关性(r=0.503,P<0.05)。结论:糖尿病对脉压增加有一定影响,脉压指数对高血压合并糖尿病患者对早期肾损伤有较好预测价值。Objective To investigate the correlation of pulse pressure index and early renal injury in senile patients with hypertension and diabetes mellitus.Methods A total of 102 patients were divided into the hypertension and diabetes group(n=52) and hypertension control group(n=50).All the patients received ambulatory blood pressure monitoring and urine microprotein detection.Pulse pressure,pulse pressure index and urine microprotein levels were compared between two groups.The correlations of pulse pressure index with urine microprotein and glycosylated hemoglobin were analyzed between two groups.Results Glucolated hemoglobin,pulse pressure,pulse pressure index and urine microprotein levels in the hypertension and diabetes group were obviously higher than those in control group(P0.05).The correlation coefficient of pulse pressure index with urine microprotein was higher in the hypertension and diabetes group than that in control group.Pulse pressure index was positively correlated to glycosylated hemoglobin(r=0.503,P0.05).Conclusion Diabetes partially influences pulse pressure.Pulse pressure index has a high predictive value to early renal injury in senile patients with hypertension and diabetes.
