不同途径经皮中心静脉置管在血液透析中的应用

摘　　要：目的比较不同途径经皮中心静脉置管在血液透析中的应用情况。方法 102例慢性肾衰竭血液透析患者随机分为两组:经皮颈内静脉置管组50例和经皮股静脉置管组52例。记录两组患者与导管相关的即刻及远期并发症,透析充分性指标尿素清除指数(Kt/V)、尿素减少率(URR)并进行统计学比较。结果经皮颈内静脉置管组较经皮股静脉置管组与导管相关的远期并发症感染率(P=0.042)及血栓形成率(P=0.027)均较低,透析充分性指标尿素清除指数(Kt/V)(P=0.046)及尿素减少率(URR)(P=0.041)均较高,与导管相关的即刻并发症两组比较差异无统计学意义(P>0.05)。结论经皮颈内静脉置管是慢性肾衰竭血液透析患者首选的临时性血管通路。Objective To investigate the application of various ways of central vein catheterization in hemodialysis.Methods 102 chronic hemodialysis patients were randomly divided into two groups：50 in internal jugular vein catheterization group and 52 in femoral vein catheterization group.Correlative acute and chronic complications,urea clearance divided by volume of distribution （Kt/V ）,urea reduction ratio（URR）were observed and calculated.Results The incidence of infection and thrombus in internal jugular vein catheterization group were lower than that of femoral group.Urea clearance divided by volume of distribution （Kt/V ）and urea reduction ratio（URR） in internal jugular vein catheterization group were higher than that of femoral group.There were no differences of the correlative acute complications in the two groups（P0.05）.Conclusions Internal jugular vein catheterization is relatively an ideal blood access for chronic hemodialysis patients.
