充血性心力衰竭患者B型钠尿肽和肌钙蛋白Ⅰ与心功能的相关性研究

摘　　要：目的探讨充血性心力衰竭(CHF)患者血浆B型钠尿肽(BNP)及血清肌钙蛋白I(cTnⅠ)与心功能的相关性,评估BNP及cTnⅠ浓度变化的临床价值。方法对明确诊断为CHF不同病因的90例患者依据NYHA心功能分级,分为心功能Ⅱ级、心功能Ⅲ级、心功能Ⅳ级3组,采用Axsym全自动免疫分析仪及其配套试剂检测血浆BNP及血清cTnⅠ浓度。结果心力衰竭患者血浆BNP浓度显著高于对照组(P<0.05),且患者血浆BNP水平与心力衰竭严重程度呈正相关(r=0.851)。血清cTnⅠ浓度虽然呈升高趋势,但与心力衰竭严重程度无明显相关性。结论心力衰竭患者随心功能分级越差,血浆BNP浓度呈递增升高,血清cTnⅠ浓度虽然呈升高趋势,但与心功能无明显相关性。Objective It is to approach the correlation of plasma type B natriuretic peptides（BNP） and cardiac troponin Ⅰ（cTnⅠ） with cardiac function in patients with congestive heart failure（CHF） and evaluate the clinical value of the concentration changes of BNP and cTnⅠ.Methods 90 CHF patients with different causes were divided into cardiac function grade Ⅱ group,cardiac function grade Ⅲ group and cardiac function grade Ⅳ group based on NYHA classification.Plasma BNP and serum cTnⅠ were detected with Axsym fully automated immunoassay instrument and supporting reagents.Results Plasma BNP concentration in heart failure patients was significantly higher than that in control group（P 0.05）.Plasma BNP level in heart failure patients was positively correlated with the severity of heart failure（r =0.851）.Serum cTnⅠ concentration had heightening tendency,but had no obvious correlation with the severity of heart failure.Conclusion Following the worse heart function,Plasma BNP concentration in heart failure patients increased progressively.Serum cTnⅠ concentration had heightening tendency,but had no correlation with heart function.
