记忆金属胸部护板在汶川地震伤中治疗多根肋骨骨折临床分析

摘　　要：目的 总结记忆金属胸部护板在汶川地震伤中治疗多根肋骨骨折的临床效果.方法 72例多根肋骨骨折伤员,采用记忆金属胸部护板行外固定（护板组）;68例采用胸带或多头腹带包扎等传统外同定方法（对照组）.观察2组伤员疼痛指数（VAS）,肺部并发症,住院天数.结果 与对照组比较,护板组伤员治疗1 h后疼痛明显好转,肺部并发症明显下降,住院天数缩短3.45 d.结论 记忆金属胸部护板同定肋骨骨折区域,临床效果满意,特别是在紧急状态下及群体伤中,是一种新型,简便实用的肋骨骨折同定方法.Objective To investigate the effects of memory metal chest shield in treatment of multiple rib fracture. Methods Seventy-two patients-victims of Wenchuan Earthquake with fracture in 3.46 ribs on average, 52 males and 20 females, aged （54.68±7.98）（19-74）, underwent treatment with memory metal chest shield, and 68 patients with fracture in 3.21 ribs on average, 45 males and 23 females, aged （56.12±8.04）（18-76）, underwent traditional external fixation, such as dressing with chest bandage or muhi-tailed abdominal bandage. Visual analog scale （VAS） was used to measaure the feeling of pain, and the prevalence rate of pulmoinary complications and hospitalization time were observed. Results The VAS score of the chest shield group at hospitalization was （9.28±0.51）, not significantly different from that of the control group [（9.31 ±0.48）, P〉0.05], and the VAS score one hour after the beginning of treatment of the chest shield group was （8.69±0.34）, significantly lower than that of the control group [（9.02±0.36）, P〈0.05], and since then the VAS scores at different time points of the chest shield group were all significantly lower than those of the control group （all P〈 0.05）. The general prevalence rate of pulmonary complications of the chest shield group was 13.8%, significantly lower than that of the control group （30.9%, P〈005）. The hospitalization time of the chest shield group was （9.20±4.12） days, signifiocantly shorter than that of the control group [（12.65±3.98） days, P〈0.05]. Conclusion Fixation with memory metal chest shield is an effective method in treatment of fracture of ribs.
