摘 要:目的总结经颅超声对诊断新生儿颅内出血(Neonates Intracranial Hemorrhage,NIH)的临床特点并评价其临床价值,探讨筛查和早期诊断NIH安全、简便的方法。方法对临床表现疑似NIH的新生儿156例进行经颅超声检查。结果 92例患儿有颅内出血超声图像改变,有窒息病史者,最常见出血部位为室管膜下出血占38例,其次为脑室出血占17例;有产伤史者,出血部位主要在硬脑膜下出血占10例,其次为蛛网膜下腔出血占5例;无明显诱因引起颅内出血者,出血部位在蛛网膜下腔者占10例,脑实质出血占6例。结论经颅超声检查诊断NIH安全、简便、无创、价廉、无射线损害、适合动态追踪观察等优点,尤其对NIH出血后随访观察,超声更具优势,超声诊断对NIH的诊断具有重要的意义,可作为常规筛查和随访NIH的首选影像学方法。Objective Transcranial Ultrasound in the diagnosis of intracranial hemorrhage(Neonates Intracranial Hemorrhage,NIH)clinical features and evaluate its clinical value of screening and early diagnosis of NIH safe and simple method.Methods The clinical manifestations of 156 cases of suspected neonatal NIH for transcranial ultrasound.Results 92 patients had intracranial hemorrhage ultrasound image change,there is a history of choking,bleeding the most common sites of subependymal hemorrhage in 38 cases,followed by intraventricular hemorrhage in 17 cases;have a history of birth trauma,bleeding sites,mainly in the hard meningeal hemorrhage in 10 cases,followed by subarachnoid hemorrhage in 5 cases;no obvious incentive to cause intracranial hemorrhage,bleeding sites in the subarachnoid space were found in 10 cases,accounting for six cases of cerebral hemorrhage.Conclusion It's safe,simple,noninvasive,inexpensive,non-radiation damage of transcranial ultrasound for diagnosis of NIH,the advantages for dynamic tracking and observation,especially for NIH were followed up after hemorrhage,ultrasound is more advantage to the NIH diagnostic ultrasound of great significance can be used as routine screening and follow-up imaging method of choice for NIH.
