颞骨HRCT对胆脂瘤型中耳炎的诊断价值

摘　　要：目的探讨胆脂瘤型中耳炎颞骨HRCT的表现及临床价值。方法回顾性分析72例胆脂瘤型中耳炎颞骨横断面、冠状面HRCT表现，并与手术结果对照。结果鼓室和／乳突窦内均有软组织影；在听小骨、鼓室盾板、鼓室天盖、面神经管、骨迷路破坏等方面，HRCT诊断的准确率分别为94．4％、91．7％、95．8％、80．6％、93．1％；特异性分别为88％、87．9％、94．4％、81．8％、95．2％；敏感性分别为97．9％、94．9％、100％、79．6％、66．7％。结论HRCT可准确显示中耳腔内软组织病灶的分布、形态及听小骨、周围重要结构的破坏情况，在胆脂瘤型中耳炎的诊断及术前评估中有重要价值。Objective To study high resolution CT（ HRCT ） findings of chronic eholesteatoma otitis media and their clinical significance. Methods HRCT findings of 72 ears with chronic eholesteatoma otitis media were analysed retrospectively and compared with surgical findings. Results All tympanic cavity and/or tympanic antrum have soft tissue mass. The accuracy of HRCT in destruction of auditory ossicles, tympanic scutum, tympanic tegmen, facial nerve canal and osseous labyrinth was 94. 4%, 91.7% , 95.8% ,80. 6% and 93.1% respectively. The specificity was 88 % , 87.9 % , 94.4% , 81.8% and 95.2% respectively. The sensitivity was 97.9%, 94. 9%, 100% ,79. 6% and 66. 7% respectively. Conclusion HRCT can accurately show location and shape of soft tissue mass of tympanic cavity and/or tympanic antrum, destruction of auditory ossieles and important structures of temporal bone, which can provide evidence in diagnosis of ehoronie eholesteatoma otitis media and assessment before surgery.
