超声心动图对CRT短期疗效的评价

摘　　要：目的应用超声心动图定量组织速度成像技术(QTVI)评价慢性心力衰竭(CHF)患者安装三腔起搏器(CRT)术后同步化治疗的短期疗效。方法选择行植入CRT的患者21例,分别在术前、术后1周及术后3个月常规测量左心室舒张末、收缩末的直径(LvDD、LVD s)及左室收缩末期容积LVESV、左室舒张末期容积LVEDV和左室射血分数(LVEF)。QTVI成像图分析左室壁包括基底段及中段12个阶段达峰时间差,并分析房室、室间、室内运动的同步性。结果与术前相比,LVEF等心功能指标及心脏同步性指标在术后1周及术后3个月均不同程度的改善。②术后1周与术后3个月各超声指标差异无统计学意义。结论 CRT术后改善了慢性心衰患者的血液动力学,心功能指标和心衰症状,逆转心室重构。Objective To evaluate short-term curative effects for patients with chronic heart failure（CHF） which installed three cavity pacemaker（CRT） by using quantitative group velocity imaging technology（QTVI）.Methods 21 patients with CRT was included in the study.The left ventricular diastolic,the end of the LvDD diameter（contraction,LVDs） and l end-systolic volume LVESV and left ventricular end-diastolic volume LVEDV and left ventricular ejection fraction（LVEF） were measured at one week before CRT operation,one week after CRT operation and postoperative three months.QTVI imaging chart analysis of left ventricular wall included basal segment and middle reaches peak time 12 stages,and analyzes the atrioventricular,room,indoor sports synchrony between.Results Comparing with preoperative cardiac index,LVEF in postoperative week synchrony cardiac index and three different degrees was improved.There was no obvious difference in the ultrasonic index after three months and after a week.Conclusion CRT postoperative improved chronic heart failure patients,cardiac hemodynamic index and failure symptom,and reverse ventricular remodeling.
