新诊断2型糖尿病患者合并动脉粥样硬化临床特征

摘　　要：目的:了解新诊断2型糖尿病患者亚临床动脉粥样硬化(AS)的发生现状,危险因素及临床特征。方法:通过无创性高分辨超声检查评价我院104例内分泌科住院的新近诊断的2型糖尿病患者的颈动脉及股动脉,评估颈动脉及股动脉内中膜厚度(IMT)及斑块。同时测定患者的体重指数、腰臀比、收缩压、舒张压、血脂谱、糖化血红蛋白、空腹血糖、早餐后2小时血糖、HOMA-IR指数。比较伴有亚临床动脉粥样硬化组与不伴有动脉粥样硬化组上述各临床指标间的差异。结果:104例患者中亚临床动脉硬化患者21例(发生率20.2%)。股总动脉检出率高于颈总动脉。AS组高血压病、高脂血症、肥胖的患病率明显高于对照组。两组间舒张压、HDL-C及HOMA-IR的差异具有统计学意义。结论:新诊断的非老年的2型糖尿病患者部分存在亚临床动脉粥样硬化;股动脉的亚临床动脉粥样硬化检出率高于颈动脉;亚临床动脉粥样硬化与胰岛素抵抗、舒张压及高密度脂蛋白有关。Objective To investigate the status of subclinical atherosclerosis（AS）,its risk factors and the clinical characteristics in newly diagnosed type 2 daibetics.Methods One hundred and four hospitalized patients with newly diagnosed type 2 diabetics were included in this study.Non-invasive,high resolution B mode ultrasonography was used to measure the intima medial thickness（IMT） and atherosclerotic plaques of common carotid artery and femoral artery in all subjects.Body mass index,waist to hip ratio,Systolic and Diastolic blood pressure,blood lipid profile,HBA1C,FBG,2hPBG,HOMA-IR were compared between with or without subclinical AS patients.Results Twenty one patients （20.2%） showed subclinical AS.More frequency subclinical AS was seen in femoral artery than in common carotid artery.Hypertension,hyper blood lipid and obesity were frequently seen in the subclinical AS patients than in patients without subclinical AS.The difference of the diastolic blood pressure、HDL-C and HOMA-IR between with and without subclinical AS groups has statistics difference（P0.05）.Conclusions Partly newly diagnosed non-older type 2 diabetics suffers from subclinical AS.More frequency subclinical AS was discovered in femoral artery than in common carotid artery.Insulin resistance,diastolic blood pressure and HDL-C were risk factors relating to subclinical AS in non-oldery patients with newly diagnosed type 2 diabetes.
