摘 要:目的检测急性特发性血小板减少性紫癜(ITP)患儿治疗前后外周血血小板相关抗体(PAIgG)、CD4+、CD25+、Foxp3+调节性T淋巴细胞(Tr细胞)的变化,探讨Tr细胞与PAIgG之间的相关性,分析Tr细胞及PAIgG在急性ITP患儿发病机制中的相互作用。方法用流式细胞仪检测外周血PAIgG和Tr细胞的数量,血细胞分析仪计数血小板(PLT)含量。结果急性ITP患儿治疗前外周血PLT数量、Tr细胞均低于健康对照组(均为P<0.05),而PAIgG含量高于健康对照组(P<0.05);随着病情痊愈或好转,血小板数量、Tr细胞含量达到正常值或接近正常值,而PAIgG含量仍然较高(均为P<0.05),PAIgG在治疗前与血小板含量呈负相关(痊愈组及好转组:r值分别为-0.81,-0.78,均为P<0.05),Tr细胞含量在治疗前与血小板含量呈正相关(r值分别为0.76、0.81,均为P<0.05),Tr细胞含量与PAIgG含量在治疗前呈负相关(r值分别为-0.64、-0.73,均为P<0.05)。结论急性ITP患儿外周血中Tr细胞数量和PAIgG含量、PLT数量均有较好的相关性,检测指标Tr细胞数量能够反映急性ITP患儿外周血中PAIgG和PLT数量的变化,Tr细胞水平的降低反映了机体的免疫功能状态降低,PAIgG及Tr细胞在急性ITP发病机制中发挥了重要作用。Objective To investigate the changes of the CD4^+ CD25^+ Foxp3^+ T lympholeukocytes, and research the variation of PAIgG in acute ITP children's peripheral blood at the same time,to approach the corraltion between Tr cells and PAIgG,to analyze the mutual actions in the pathogenesy of acute ITP furtherly. Methods To detect the quantity of the CD4^+ CD25^+ Foxp3^+ T lympholeukocytes and PAIgG and count the number of platelet (PLT) with blood cell analyzer. Results The percentage of Tr and amount of platelet were obviously lower in ITP children's peripheral blood than the healthy control group before the treatment(P〈0.05) ;On the contrary,the concentration of PAIgG was higher than the healthy control group before the treatment(P〈0.05), as the disease was cured or improved, the number Of platelets and Tr cells reached the normal, but PAIgG was still higher (P〈0.05), PAIgG was negatively correlated with platelet content before treatment (Recovery group and Improved Group.. r values were -0.81, -0.78 respectively, both P〈0.05), Tr was positively correlated with the platelet concentration before treatment (r values were 0.76, 0.81 respectively, all P〈0.05), there was a negative correlation between Tr and PAIgG content before treatment (r values were -0.64, -0.73 respectively, both P〈0.05). Conclusion Tr have better correlations with PAIgG and the amount of PLT in peripheral blood of acute ITP children . The detection of Tr can be indicators to reflect the the number of PAIgG in peripheral blood, and reduction of Tr level reflected the decreased immune function of the state in acute ITP . PAIgG and Tr play important roles in the pathogenesis of acute ITP.
