摘  要:目的探讨HCV感染与糖、脂代谢紊乱的相互关系,为慢性丙型肝炎合并非酒精性脂肪肝患者的预后判定及合理诊治提供依据。方法对确诊的75例慢性丙肝患者糖、脂代谢指标及HCV抗体及HCVRNA载量进行检测,并与75例健康体检者进行对照。结果 75例HCV阳性患者中,合并非酒精性脂肪肝的25例,患病率为33.3%。与健康对照组比较,两者差异有统计学意义(P<0.01)。HCV合并NAFLD组的TC、TG、ALT、TBA、FBG、GGT与单纯HCV组比较,差异有统计学意义,尤以TG、ALT升高明显(P<0.01),而AST、TBIL、LAP、HDL、LDL等水平差异无统计学意义(P>0.05)。HCV病毒载量越高,合并NAFLD的可能性越大。结论 HCV感染合并NAFLD可促进肝纤维化的发生,并影响抗病毒疗效,肝脂肪变性及其伴随肥胖的控制有助于改善肝功能、提高抗病毒治疗效果并阻止肝纤维化进展。Objective To investigate the relationship between HCV infection and glucosee , I.ipid disorders, to supply evidence of prognosis and diagnosis in patients with chronic hepatitis C combine to non-alcoholic fatty liver diseas. Methods The level of metabosim marker of glucose and lipid , the antibody of HCV and the copies of HCV were measured in 75 patients with Chronic Hepatitis C , while 75 healthy people were referred as control group. Results Of the 75 patients with HCV,25(33.3%)were with non-alcoholic fatty liver diseas ,and the ratio was significantly higher than that of the control group(P〈C0.01), in the group of HCV combine to NAFLD, the level of TC, TG. ALT. TBA. FBG, GGT were significantly higher than that of the HCV group(P〈 0.0]) ,and the level AST, Tbil, LAP, HDL,LDL showed no difference. And the copy of HCV was higher , the possibility of combined NAFLD was more. Conclusion HCV infection combine to NAFLD may promote development ,and the effect of anti-virus, to control fatty may conduce to Hepatic function, and Treatment Effectiveness of anti virus. And to prohibit the progression of hepatic fibrosis.
