血管周围间隙的影像诊断

摘　　要：目的探讨血管周围间隙(VRS)的CT、MRI特征,以提高诊断符合率。方法随机选择98例经磁共振(MR)检查发现的VRS患者并对其中16例进行了MR增强扫描,19例行MR弥散加权成像(DWI),38例行CT检查。结果本组患者VRS均为双侧多发、呈圆形或椭圆形点状、条状或线状图像,边界清晰锐利。条形或线形的血管周围间隙多成簇出现,且沿纤维方向排列。发生部位包括基底节前联合水平(基底节下1/3)(100%,98/98)、近大脑凸面的半卵圆中心(87.8%,86/98)、脑干大脑脚处(36.7%,36/98)、外囊(23.5%,23/98),小脑齿状核旁白质(2%,2/98)。CT图像上仅能分辨大血管周围间隙,呈水样低密度,增强扫描无强化;MRI不同序列显示间隙信号与脑脊液信号完全一致,即T1WI及FLAIR像呈低信号、T2WI表现为高信号、DWI为低信号;增强扫描无强化。结论明显扩张的血管周围间隙影像学表现具特异性,易于和其它颅内腔隙灶鉴别。Objective To analyze the CT and MRI imaging features of perivascular spaces（Virchow-Robin Spaces,VRS） to promote the diagnostic coincidence.Methods MR images of 98 cases of VRS were selected randomly in this study.Among them,16 cases underwent enhanced MRI,19 case underwent diffusion weighted imagingand and 38 cases received routine CT examination.Results Multiple VRS were found in cerebral and cerebellar hemispheres bilaterally,round or oval,punctual,strip or linear with clear borderline in all.The lesions shaped strip or linear usually clustered and arrayed along the white matter fasciculi.Their location was consistently in around anterior commissure（100%,98/98）,centrum semiovalnearhigh-convexity of the cerebrum（87.8%,86/98）,cerebral peduncles（36.7%,36/98）,extreme cap sules 23.5%,23/98）,and cerebellar white matter（2%,2/98）.Only the bigger perivescular spaces can be found in CT imaging,with low density and no enhancement after injection of contrast agent.The signal of perivescular spaces was isointens relative to CSF on all MR pulse sequences,and with no enhancement after injection of contrast agent.Conclusion Unusual widening of VRS in the brain has some characteristic manifestations on MRI and CT.
