髋臼后壁骨折伴股骨头脱位手术效果分析

摘　　要：目的:进一步提高髋臼后壁骨折(PWFA)伴股骨头脱位的手术效果。方法:对42例PWFA伴股骨头脱位患者先行手法复位,然后行髋臼后壁骨折复位内固定术治疗,对影响因素进行统计对比,分析其手术效果。结果:术后随访1～3年,临床疗效Harris标准评分,优良率为83.3%,其中,伤后股骨头复位≤6h优良为30/33,>6h优良为3/9;伤后至手术时间≤10d优良为33/37,>10d优良为2/5;骨折复位质量≤3mm优良为30/32,>3mm优良为5/10;钢板固定优良为31/33,螺钉固定优良为4/9,以上4组经统计学分析P<0.05。结论:重建钢板内固定术治疗PWFA伴股骨头脱位效果确切,6h之内进行解剖复位并在10d内手术效果更好。Objective：To further improve the posterior wall acetabular fractures （PWFA） the effect of surgery with femoral head dislocation.Methods：42 patients with femoral head dislocation PWFA manipulative reduction first,then underwent posterior wall acetabular fracture reduction and internal fixation,a statistical comparison of the factors to analyze the effect of surgery.Results：The patients were followed up 1 to 3 years,Harris scores of clinical efficacy,excellent rate of 83.3%,of which the femoral head injury reduction ≤6 hours after the fine was 30/33,6 hours good 3/9;from injury to surgery time ≤10 days good as 33/37,10 days good 2/5;fracture reduction and fine quality of ≤3 mm for the 30/32,3 mm high and 5/10;good for the 31/33 plate,screws good 4/9,more than 4 groups by statistical analysis P0.05.Conclusion：The reconstruction plate fixation PWFA exact effect with femoral head dislocation,6 hours and within the anatomical reduction surgery in the 10 days period better.
