妊娠期高血压疾病患者胎盘中VCAM-1的表达及其临床意义

摘　　要：目的探讨妊娠期高血压疾病(HDCP)患者胎盘组织中血管细胞粘附分子-1(vascular endothelial cell adhesionmolecule,VCAM-1)的表达及其临床意义。方法应用免疫组化Envision法检测了38例HDCP患者及22例正常妊娠晚期胎盘组织中VCAM-1的表达。结果 VCAM-1主要表达在胎盘滋养叶细胞及血管内皮细胞;VCAM-1在HDCP患者胎盘滋养叶细胞表达率为21.05%,低于正常妊娠晚期的63.64%,差异性显著,有统计学意义(χ2=10.86,P<0.005)。而在血管内皮细胞表达率为44.74%,高于正常妊娠晚期的31.82%,无显著差异性,无统计学意义(χ2=0.97,P>0.05)。结论 HDCP患者胎盘滋养层细胞缺乏VCAM-1表达可能与HDCP发生发展有关。Objective To discuss the expression of the vascular endoth-elial cell adhesion molecule-1（VCAM-1）on the placenta tissue in the case of hypertensive disorders complicating pregnancy（HDCP） and its effect on the start and development of HDCP.Methods The expression of VCAM-1 in the placenta tissue between 38 HDCP patients and 22 normal ones in later period of pregnancy was detected by means of immunohistochemistry.Results VCAM-1 was mainly expressed in the placental cytotrophoblasts and the endovascular cells;VCAM-1′s expressive rate of placental cytotrophoblasts in HDCP patients was 21.05%,which was lower than that （63.64 %） in normal ones in later period of pregnancy.There was significant difference（P0.05）.The expressive rate of endovascular cells in the HDCP patients（44.74 %） was higher than that（31.82 %） in normal ones in later period.There was no significant difference（P 0.05）.Conclusion It was possible that there was some relationship between the start and development of HDCP lacking of the VCAM-1 expression in the placental cytotrophoblasts of HDCP patients.
