云克联合硫酸氨基葡萄糖治疗膝骨关节炎骨髓水肿临床观察  被引量:3

摘  要:目的观察云克联合硫酸氨基葡萄糖治疗膝骨关节炎(OA)骨髓水肿(BME)的近期临床疗效。方法选择核磁共振(MR I)提示有BME的膝骨关节炎患者80例,随机分为治疗组40例,对照组40例。两组均口服硫酸氨基葡萄糖6个月,治疗组加用云克静脉注射,每次2套,每日一次,连用10 d休息10 d为一个疗程,共三个疗程。采用Lysholm评分标准评估膝关节功能;依据Felson等对BME的评估方法对膝关节的BME进行评分。结果治疗前两组膝关节功能Lysholm评分及BME积分均无差异(P>0.05);治疗组在治疗3月、6月时膝关节功能评分明显高于对照组(P<0.05),而膝关节BME积分显著低于对照组(P<0.05)。两组因药物副作用停药者差异无统计学意义(P>0.05)。结论云克联合硫酸氨基葡萄糖治疗膝骨关节炎BME疗效显著,安全性好,值得推广应用。Objective To observe the clinical effect of ^99Tc-MDP and glucosamine sulfate on osteoarthritis(OA) of knee with bone marrow edema(BME).Methods 80 patients of knee osteoarthritis with BME were randomly divided into treatment group(40) and control group(40) taking glucosamine sulfate for 6 months respectively.And the patients of control group were treated by ^99Tc-MDP with 2 sets once per day on 3 course treatments.Then knee function is assessed by using Lysholm scoring standard,obtained is the knee′s score of BME according to Felson′s assessment methods.Results The Lysholm scoring and integral score of BME of the knee were not significantly different before the treatment(P〈0.05);the treatment group was significantly higher than that of the control group in knee function score after receiving treatment after 3 months and 6 months(P〈0.05),while the BME of knee was significantly lower than that of the control group(P〈0.05).There was not significant difference on side effect in these groups(P〉0.05).Conclusion ^99Tc-MDP combined glucosamine sulfate can be put to further application on the treatment of osteoarthritis of the knee with BME for its significant effect with few side effects.
