摘 要:目的:研究缺氧诱导因子-1α(HIF-1α)、碳酸酐酶Ⅸ(CAⅨ)在鼻息肉组织中的表达及其相关性,探讨HIF-1α、CAⅨ在鼻息肉发病过程中的作用。方法:收集28例鼻息肉组织(鼻息肉组)和12例正常下鼻甲组织(对照组),用免疫组织化学法检测鼻息肉组与对照组中HIF-1α与CAⅨ的表达,观察比较2组之间表达的差异。结果:免疫组织化学染色显示鼻息肉组HIF-1α、CAⅨ免疫阳性细胞数及着色强度均明显高于对照组。图像分析显示,鼻息肉组HIF-1α、CAⅨ的积分吸收度值(103/HP)分别为21.76±3.52、26.87±4.60,与对照组3.37±1.65、3.25±1.20相比差异有统计学意义(P<0.05);Spearman等级相关分析显示,鼻息肉组中HIF-1α表达与CAⅨ表达密切相关(r=0.820,P<0.01)。结论:鼻息肉组中HIF-1α、CAⅨ表达均上调,且二者有协同表达关系,说明鼻息肉组织中存在缺氧现象,再加上感染、炎症等作用使氧耗增多,能量代谢失调,加重局部缺氧,HIF-1α表达升高,并且能够激活、上调CAⅨ的表达,从而在缺氧状况下维持细胞的正常pH值,使细胞适应缺氧微环境,从而保证缺氧区域细胞的持续生长和增殖,最终形成恶性循环,导致鼻息肉的形成。Objective:To analyze the expression and relationship between hypoxia inducible factor-1α(HIF-1α)and carbonic anhydrase Ⅸ(CA Ⅸ)in nasal polyps,and to explore the role of HIF-1α and CA Ⅸ in the pathogenesis of nasal polyps.Method:Twenty-eight tissues of nasal polyps and 12 tissues of normal inferior turbinate mucosa were collected in this study.And Immunohistochemistry method was used to detect the expression of HIF-1α and CA Ⅸ in the two groups.Result:The numbers and intensity of positive stained cells of HIF-1α and CA Ⅸ were higher in nasal polyps.IOD of HIF-1α and CA Ⅸ(103/HP)in the nasal polyps were respectively 21.76±3.52,26.87±4.60 compared with 3.37±1.65,3.25±1.20 in the control group by image analysis.There were significant differences between the two groups(P0.05).Spearman correlation test showed that there was a close correlation between the HIF-1α and CA Ⅸ expression in nasal polyps(r=0.820,P0.01).Conclusion:The results showed that HIF-1α and CA Ⅸ were over expressed in nasal polyps,and there was a close correlation between them.Hypoxia was found in nasal polyps and it could become more severe under infection,inflammation and energy metabolic disorder which consumed a great deal of oxygen.HIF-1α was over expressed in nasal polyps,which activate and promote the expression fo CA Ⅸ,in order to keep the normal pH of cells for adapting the hypoxic microenvironment and sustain persistent growth and hyperblastosis of cells in the hypoxic areas.At last the infernal circle was forming,which accelerate the formation of nasal polyps.
